Job Overview:We are seeking a dedicated and compassionate Patient Care Coordinator to join our healthcare team in Grass Valley, CA. This full-time position offers a dynamic opportunity to work closely with patients, physicians, and medical staff to ensure a smooth and efficient patient experience. As a Patient Care Coordinator, you will play a critical role in facilitating patient appointments, managing schedules, and providing exceptional service to our community.
Key Responsibilities:
Patient Scheduling & Coordination: Schedule patient appointments, confirm appointments, and follow up as necessary to ensure smooth coordination of care.Patient Communication: Serve as the main point of contact for patients, addressing inquiries, providing information, and ensuring patient satisfaction.Documentation & Data Entry: Accurately maintain patient records in the electronic health record (EHR) system, ensuring all information is updated and compliant with healthcare regulations.Insurance Verification & Billing: Verify patient insurance coverage, assist with pre-authorizations, and collaborate with the billing team to address any insurance or payment concerns.Care Coordination: Assist in coordinating care among various healthcare providers, ensuring patients are informed about referrals, tests, and procedures.Patient Advocacy: Advocate for patients by providing resources and support, answering questions, and helping them navigate their healthcare needs.Reception Duties: Greet patients, manage patient check-ins and check-outs, and ensure a positive and welcoming environment.Follow-Up: Ensure patients are contacted post-appointment for follow-up care, including reminders for additional treatments or check-ups.Collaboration: Work closely with medical providers, nursing staff, and administrative personnel to ensure high-quality care and patient satisfaction.Qualifications:
High school diploma or equivalent (required); Associate's or Bachelor's degree in healthcare or related field (preferred).Minimum of 1 year of experience in a healthcare setting, preferably in a patient coordination or administrative role.Strong communication skills, both written and verbal.Proficient with Microsoft Office Suite and experience with EHR systems.Knowledge of HIPAA regulations and healthcare privacy laws.Excellent organizational and multitasking abilities.Compassionate, patient-focused, and detail-oriented attitude.Ability to handle sensitive patient information with discretion and professionalism.Ability to work independently and as part of a team.
