Job DescriptionCoders take a physician's narrative and translate that information into ICD-10-CM & ICD10 PCS (International Classification of Diseases, 10th Revision, Clinical Modification & Procedural Coding System) and CPT (Current Procedural Terminology) codes.The coder must:Code inpatient records and MRS code outpatient records.Have the ability to abstract pertinent information from Emergency Room, Ambulatory Surgery and Ambulatory Care records and assign ICD-10-CM diagnostic & ICD-10-CM Procedural codes and CPT codes.Have knowledge of APGs & APCs and DRGs (for Inpatient Coding).Code accurately, completely, and consistently according to coding principles and guidelines so that reimbursement and compliance is not compromised.Have excellent communication skills in order to query physicians when code assignments are not straightforward or documentation in the record is inadequate, ambiguous or unclear for coding purposes.Have knowledge ofthe official coding guidelines as set forth by the American Hospital Association (AHA), American Medical Association (AMA), American Health Information Management Association (AHIMA) and other governing bodies; and reimbursement reporting requirements.Have computer skills with the ability to navigate and utilize the Electronic Medical Record system to facilitate coding.
Minimum Qualifications1. A Bachelor's Degree in Medical Record Administration with at least six months experience as a Medical Record Specialist or Medical Record Technician; or,2. Completion of a course of study for Medical Record Technicians approved by the American Medical Record Association and one year experience as a Medical Record Specialist or Medical Record Technician with six months of supervisory experience; or,3. A certificate issued upon completion of the Correspondence Course for medical record personnel given by the American Medical Record Association plus the experience listed in (2); or,4. High school graduation or evidence of having passed an examination for the High School Equivalency Diploma or the U.S. Armed Forces G.E.D. certificate with a score of at least thirty-five on each of the five tests and an overall score of at least two-hundred-and-twenty-five in the examination for the certificate and three years experience as a Medical Record Specialist or Medical Record Technician with one year of supervisory experience; or,5. A satisfactory equivalent of education and experience.6.
