Summary:Under the supervision of the Office Manager and the Human Resource Associate the Medical Records Clerk will be responsible for managing and maintaining clients' medical records, ensuring accuracy, confidentiality and the legal release of information. Responsibilities/Essential Functions: Organizes the condition of client paper charts by thinning, scanning and purging. Converting clients' paper charts to electronic health records. Assist in the preparation of reports and documentation as needed for audits or compliance purposes. Ensure compliance with all applicable laws and regulations regarding client confidentiality and data security when handling and storing PHI. Collaborate with clinical staff to resolve discrepancies or issues related to medical records. Responds to medical recorded requests from clients or third-party providers while adhering to privacy regulations. Communicate with clients to provide updates to requests or concerns around their medical records. Writes, types, or enters information into computer or log to prepare correspondence and accurate accounting of information releases. Maintains copy machines, scanner and fax machine and assist staff with problems or call service if needed. Back up other office staff with receptionist coverage of front desk and switchboard when needed. Adheres to clients' rights under the Mental Health Code, Confidentiality Act, HIPAA and Rule 50 of the Office of Inspector General, abides by Stepping Stones Code of Ethics and Professional Practice.Attends all required training and staffing.Adheres to Stepping Stones' Policies and Procedures.Performs other duties as assigned.Contacts:The Medical Records Clerk has contact with all Stepping Stones employees and clients. Because this position also performs many receptionist duties, they also encounter clients' family members, health care professionals from other agencies and various venders. Supervisory Responsibilities:Not applicable.RequirementsQualification Requirements:To perform this job successfully, an individual needs to be very organized and possess an eye for detail. They must be multi-tasked, flexible and have good people skills. Ability to maintain confidentiality and handle sensitive information responsibility. Familiarity with medical terminology and understanding of medical records management is essential. Proficiency in using electronic health record (EHR) systems and other relevant software applications. Previous experience in a medical office setting is preferred. Education: Minimum requirement: High school graduate or equivalent. Qualified employees should have some general computer knowledge and have worked in an office environment for at least 2 years. Physical Requirements: While performing the duties of this job the employee is frequently required to sit up to 4 hours, stand up to 4 hours, squat, bend, carry with two hands, repeatedly lift 5lbs.with one hand, operate switchboard, walk, stamp, write, use phone and file. Occasionally this employee will be required to lift to 25 lbs. walk up and down stairs. They must be capable of hearing at normal speaking levels and talking over the phone and in person. This job is rated as medium in its overall level of physical activity. Reasonable accommodation may be made to enable individuals with disabilities to perform the essential functions. Work Conditions:The front office is moderately noisy and busy with frequent interruptions from staff, clients, visitors and phones. The Medical Records Clerk works from 8am to 5pm day and a 40-hour workweek. There is risk involved with client contact.
