As a Medication Aide at our Long Term Care facility, you will provide direct, hands-on resident care, with all activities of daily living. You will serve a crucial role in the lives of our residents and as a part of our team. You will be responsible to observe and report changes in resident status to the nursing team. You will share personal times through support celebrating holidays, birthdays, anniversaries, and the birth of grandchildren. Job Responsibilities:
- Utilizes individual care plans of residents for the performance of resident care.
- Administer medications as prescribed by physician and record on the M.A.R.
- May perform inhalation therapy, which includes inhalers and nebulizer. Oxygen may be given by inhalation. Apply topical applications of sprays, creams, ointments, lotions and transdermal patches. Instillation of eyes, ears, and nose medications.
- Take vital signs or observe residents to detect response to specific medications and report findings to charge nurse or licensed staff
- Attends to individual needs of residents, which may include assistance with grooming, bathing, oral hygiene, feeding, incontinent care, toileting, colostomy care, and prosthetic appliance.
- Participates in creating and following through with the specific care plan of each resident, i.e. all ADL's, and post mortem care. Observes and reports any change in resident's vital signs or status to nurse caring for resident or charge nurse.
- Serve and collect food trays, pass water and assist residents with feeding who require assistance. Accurately record a resident's intake and output.
- Assist/ perform all protocols of SRMC admitting procedures for new residents.
- Assist in setting up auxiliary equipment (i.e. oxygen, suction, Gomco, nebulizers, K-Pads, ect.) under the direction of the supervising nurse. Keep resident rooms neat, clean, and free of obstructions. Apply compresses, ice bags, aqua-pads, and Ted hose.
- Performs transferring, ambulation, or range of motion as directed in individual care plans.
- Lifts, moves and transports residents, using proper body mechanics or lifting devices.
- Performs various tasks assigned by the charge nurse, including checking vital signs, weighing residents, and collecting specimens.
- Answers call lights promptly and courteously.
- Provides care that maintains each resident's skin integrity to prevent pressure ulcers, skin tears and other damage by changing incontinent residents, turning, repositioning immobile residents and applying moisturizers to fragile skin and other areas.
- Walks extensively to and from various locations of the unit while making rounds and transports residents within the facility.
- Demonstrates personal accountability in the delivery of resident care in a competent, professional, and compassionate manner.
- Assists in escorting residents for appointments or attending activities or church services.
- Performs and maintains documentation.
- Reports to the Charge nurse and records observations and care given, to determine changes in residents.
- Verify identity of residents receiving medications and record name of drug, dosage, route, and time administered in the M.A.R.
- Document in the M.A.R. reasons why a prescribed drug was not administered and notify Charge nurse.
- Completes shift end check with a nurse prior to leaving duty.
- Conducts and document through the EMR a thorough account of each resident's status during scheduled shift.
- Communicates effectively.
- Communicates and collaborates effectively with other health team members regarding resident condition, nursing plan of care, resident satisfaction needs and recommendations for meeting identified outcomes.
- Follows approved shift report format when communicating resident information.
- Identifies and recognizes abnormal symptoms/changes in resident's condition, established priorities, and takes appropriate action. Appropriately reports condition changes to charge RN or appropriate license nurse.
- Communicates and interacts effectively and tactfully with the residents, visitors, families, peers and supervisors.
- Will report any suspected or known illegal activities.
- Promotes and ensures resident safety in performance of all responsibilities.
- Assist in maintaining a safe, neat and clean environment; reports environmental deficiencies to the charge nurse such as lighting and equipment.
- Demonstrates proficient technical/clinical skills and operational knowledge of equipment on the floor.
- Informs and involves licensed staff regarding resident care issues in a timely and appropriate manner.
- Utilizes proper body mechanics, transfer/lifting techniques and appropriate equipment to minimize fall risk to resident and injury to self.
- Protects the personal belongings of each resident, including eyeglasses, dentures, hearing aids, furnishings, jewelry, clothing and memorabilia. Promptly reports missing items according to established policy and participates in efforts to locate missing items.
- Promotes a homelike environment for all residents.
- Follows HIPAA regulations and ensures that the confidentiality of resident's medical, personal, financial records, employee records, and organizational records is maintained.
- Complies with the state and federal regulatory requirements of the company Compliance
- Presents self in a professional manner and enhances professional growth and development through participation in education programs, current literature, in-service meetings and workshops.
- Other duties as assigned.
Education, Experience, & Licensure:
- Education: Nurse Aide with 76 training hours and 40 hours of pharmacology education.
- Education: Prefer geriatric experience, but will provide on-the-job training.
- Required licensure: State of Nebraska Medication Aide
- Automated External Defibrillator (AED) training.
