Job Description:

Medication Assistant Professional (MAP) will oversee the administration of Medications to the individuals served at Vanderheyden OCFS and OPWDD Programs in accordance with New York State of Children and Family Services, OPWDD as well as the policies and procedures of Vanderheyden. The MAP will be responsible for administering medications and all medication documentation required per agency policy and procedure. Primary Duties & Essential Functions: A. Medication Administration: 1. Comply with requirements of 291 License. 2. Compliance with applicable laws, rules, and regulations to include OPWDD and OCFS Guidelines. 3. Main responsibility of MAP will be medication administration. 4. Must not come out of parameters to do MAP position. 5. Must not have been written up for medication error within last year. 6. Review the medication Administration records to ascertain that they accurately reflect the medication prescription and that the documentation of medication is completed correctly. 7. Must not have needed to return to medication class due to error in last year. 8. Make sure Medication Administration Record's (MAR's) have been double checked and verified by nurse. 9. Must be able to medicate 2-3 cottages per shift at RTC and per program in CSD. 10. Must have OCFS and OPWDD clearance per HR. 11. Must have current medication pour (within last year). Must maintain yearly re-certification requirements. 12. To be med cert for at least 6 months preferred. 13. Administer medication to Individuals during the morning and evening. 14. Buddy direct support staff who have administered medication. 15. Hours are scheduled per Program needs: 7:00AM to 9:00AM 4:00PM to 6:00PM 8:00PM to 10:00PM 16. Reports to the Director of Health Services or designee. Required Education, Knowledge, and Skills: • Promotes and supports cultural diversity and competence and is strength based. • Maintain confidentiality in accordance with agency policy and HIPPA regulations. • Participate in required in service trainings. • Attend educational trainings, workshops or conference when available. • Will possess a high school diploma or GED. Abilities and Working Conditions: • Must be able to lift 25 pounds. • Must be able to stand for moderate periods of time. • Must have a valid driver's license. • Willingness to respond to the needs of culturally diverse population. RTC - Wynantskill campus only. Community Services - travel may be required between locations. Applying internally, you must enter your education and experience.
