PennsylvaniaThe Medication Therapy Management Specialist focuses on optimizing medication therapy for patients through comprehensive reviews, education, and collaboration with healthcare teams.
Key Responsibilities:- Conduct thorough medication reviews and assessments to identify potential drug-related problems.
- Develop and implement individualized medication therapy plans in collaboration with patients and healthcare providers.
- Provide patient education on medication usage, adherence, and lifestyle modifications.
- Monitor patient progress and adjust therapy based on clinical outcomes.
- Document and report medication-related interventions and outcomes.
Requirements
- Doctor of Pharmacy (Pharm.D.) degree with training in medication therapy management.
- Board certification in medication therapy management (BCMTMS or equivalent) preferred.
- Strong analytical and clinical skills.
- Effective communication and interpersonal skills for patient and provider interactions
