We are part of a beautiful residential neighborhood, near to highways and public transportation with easy access to both Philadelphia as well as the Jersey Shore.The CNA: Certified Nursing Assistant performs various non-professional nursing care activities and related services necessary to care for the personal needs and comfort of the residents.Requirements for a CNA: Certified Nursing Assistant:High School Diploma or GED6 months experience in a long term care or assisted living settingCurrent and valid CNA certification in New JerseyResponsibilities for a CNA: Certified Nursing Assistant:Perform personal care activities for residentsCompletes daily skin checks and reports changes to nurse immediatelyParticipates in the dining experience, i.e., serving, clearing, cutting & feeding- based on residents' strengthsShares resident preferences with other staff using appropriate communication methodsMaintains continuity of care daily by timely documenting and communicating care tasks completed with each individual resident assigned in the point of care kioskAttends and actively participates with Interdisciplinary Team Care Plan meeting to identify residents' strength based approaches and skills for assigned residentsAssist residents with the use of adaptive equipment, including but not limited to: walkers, wheelchairs, eyeglasses, dentures and hearing aidsUnderstands, uses and follow restorative nursing principles and practices to assist resident in maintaining their optimal level of function and maximize their strengthsReads and understands the individualized summary plan for residents, and provides support to the residents according to their summary and care planObserves and identifies residents' changing needs and condition at first indication, and reports immediately to nursing as appropriateTakes and records vital signs & weights as instructed by nurse.
