Come join our life-changing team building hope, resilience and well-being as a Mental Health Case Manager!Shift Details: Full-time - 40 hours/weekCompensation: $44,000-$54,500 annual salary - based on experienceLocation:2700 1st Street N, Suite 201, St. Cloud, MN 56303 - includes travel throughout Central MNAbout the job:VOA is hiring a Mental Health Case Manager to provide coordination of services to support adults and children with mental illness and their families throughout Central Minnesota (Benton, Stearns, Sherburne, Wright, Chisago, Isanti, and Mille Lacs). Case Managers help coordinate services to support areas such as mental health, social, education, health, vocation, recreation, volunteer, advocacy, and transportation. Essentials:Provide services in the four core case management components-Assessment, Service Plan Development, Referral and Linkage and Monitoring, and Coordination of Services.Coordinate with a mental health professional to assure the completion of a mental health assessment of client needs and to develop individualized plans.Develop and update the client's individualized plan and functional assessment.Coordinate, monitor, and evaluate services that are outlined in the plan for appropriateness, progress, and continued service need.
