Responsibilities:
- Support the clinical team in evaluating the needs of youth and families by utilizing therapeutic interventions to foster positive outcomes.
- Collaboratively design, execute, assess, and modify individualized care plans as part of a multidisciplinary team, addressing each client's specific needs.
- Coordinate and facilitate connections to programs and available resources provided by the care team.
- Offer individual, group, and family therapy based on client requirements. Partner with informal supports, community organizations, and mental health professionals to ensure clients receive necessary services.
- Maintain accurate and timely documentation, including but not limited to clinical evaluations, treatment plans, and progress reports. Serve as the main point of contact for youth treatment plans.
- When necessary, provide transportation for youth to appointments, meetings, or other engagements.
- Uphold all policies, procedures, and relevant government regulations.
Requirements:
- Master's degree in Social Work or Mental Health Counseling with a valid NYS Professional license.
- Minimum of 1+ years of relevant clinical experience working with youth and families facing mental health challenges.
- Thorough understanding of provided services, strong advocacy skills, and knowledge of payment processes.
- Must possess a valid driver's license and meet agency driving standards.
- Licensed or permit eligible upon hire.
