Job Summary and Qualifications

GENERAL SUMMARY OF DUTIES: Gathers, collects, and disseminates information in the Intake Department. Collaborates with and contributes to the interdisciplinary team's plan of care. Identifies and utilizes appropriate resources to optimize effective, efficient referrals and placements to other inpatient facilities and outpatient discharge planning. Must be flexible and adjust well to the needs of a fast paced environment.

DUTIES INCLUDE BUT NOT LIMITED TO: Supports department by responding to telephone inquiries from psychiatric facilities, providers, and insurers related to the care of behavioral health patients. Facilitates safe and timely dispositions, works to decrease length of stay, prevent readmissions, and reduce patient, family, physician or Hospital exposure to legal and financial risk. Ensures good communication by reporting to community/state agencies any patient issues as required by law. Under the direction of the physician, facilitates admissions to Copestone and other inpatient facilities including the completion of all necessary documentation and commitment paperwork, and maintains up-to-date knowledge of community resources in order to provide appropriate discharge planning when appropriate. Enhances professional growth and development by participating in related activities.

EDUCATION: (Required)Bachelors degree in social work, counseling, psychology, or other equivalent degree in a recognized mental health field. (Preferred) BSW/LCSWA

EXPERIENCE: (Required) 3+ years experience in health care setting or local social service agency. BSW/MSW internship in hospital setting accepted. (Preferred) 5+ years hospital/healthcare social work and discharge planning experience. An equivalent combination of education and experience considered.
