Now through September 2024, CFF is offering a $500 sign-on bonus*; $250 paid on the first check and $250 on the paycheck following 6 months.*Bonus amounts could be prorated based on number of billable hours worked.Immediate opening for a Full-Time Mental Health Facilitator for our Bucks County High Fidelity Wraparound Family Teams Program.The High-Fidelity Wraparound process; known in Montgomery County as High Fidelity Family Teams, is a needs based planning process to improve the lives of children with complex mental health needs and their families by empowering the families and youth to have their voices heard through an individualized planning process.The individualized plan is child-centered and family-focused with the process focusing on strengthening the natural family, extended family and social supports for the child and family by developing a team to support the family throughout the process and in learning the skills of the model.Responsibilities:The Mental Health Facilitator will collaborate with the youth and their family to engage them in the High Fidelity Family Teams process and help them to identify and express their needs.Collaborate with other services providers and natural support involved with the youth and family while managing the team process.Ensure that the team mission, goals, and plan address the youth and family's needs.Monitor implementation of the plan and communicating frequently with the HFW Team who support the youth and family to identify any emerging needs that may need to be added to the plan.Teach and transfer the High Fidelity skills to the youth, family and natural supports by modeling, building and monitoring the youth and family's self-efficacy.
