POSITION TITLE: ACT Team Lead/Mental Health Professional

DEPARTMENT: Assertive Community Treatment Team (ACT)

GENERAL HOURS

OF WORK: Eight hour shifts- typically Monday-Friday 8am-5pm. Some occasional after hours work may be necessary.

EXEMPT/NON EXEMPT: Exempt

SUPERVISOR: Director of Operations

POSITION SUMMARY: Under the supervision of the Executive Director and Psychiatric Care Provider; supervises, administers and directs the Assertive Community Treatment Team Program in providing comprehensive assessment, planning, treatment, education and support for recipients of ACT Services and the ACT Team. Is an active member of the self contained clinical Assertive Community Treatment Team providing transdisciplinary rehabilitative and support services to identified clients with severe and persistent mental illness. All of these areas may involve providing assessment and crisis interaction to acute, chronic or moderately disturbed individuals experiencing mental distress or dysfunction.

SUPERVISION RESPONSIBILITIES:

* In conjunction with the Director of Operations and ACT Team Psychiatrist, provides lead mental health professional functions including clinical and administrative supervision of identified ACT Staff.

* In conjunction with the Director of Operations, is responsible for retaining and managing performance of identified ACT Team Staff.

* Functions as ACT Team primary liaison with County Family Services and other agencies/entities.

* Assists in ensuring all policies, procedures and practice guidelines meet applicable payor, government, contractual, licensing and applicable code of ethics rules and guidelines.

* Provide effective oversight, direction, and clinical and administrative supervision to assigned staff/programs.

* Ensure effective 24-hour, 7 day a week, 365 day a-year coverage for individuals as required by program.

* Lead meetings and disseminate information to other ACTT staff from various sources. Provide support to ACTT staff.

EDUCATION, EXPERIENCE AND SKILLS REQUIRED:

* A minimum of a Master's degree in a behavioral science field from an accredited University/College.

* State of Minnesota, PsyD, LMFT, LICSW, CNS, or LPCC Licensure or Licensable within 24 months of hire date.

* Must by 21 years of age.

* Proven supervisory skills.

* Proven clinical programming skills.

* Must possess a valid Driver's License and Auto Insurance.

* Experience working with adults diagnosed with serious and persistent mental illness.

ESSENTIAL FUNCTIONS:

* Performs all duties in accordance with the policies and procedures of the Center and ACT Team, and adhere to all compliance measures.

* Participates as requested in staff recruitment, interviewing, hiring, work assignments, orientation, and performance supervision according to work rules, regulation, and policies; develops and implements staff orientation, training and manages student training.

* Directs and coordinates the client admission process and treatment, rehabilitation and support services of the program in coordination with the psychiatrist.

* Manages admissions and discharges to the ACT team according to established protocols.

* Schedules the admission interview; develops and coordinates the initial assessment and initial treatment plan; assigns the most appropriate staff to the individual treatment team; and provides clinical supervision of the development of the comprehensive assessment and treatment plan for each client.

* Directs and coordinates, for each client, the client-centered comprehensive assessment of psychiatrist history (e.g., onset, course, and effect of illness, past treatment and responses, and risk behaviors), mental status, and diagnosis; physical health, and dental health; use of alcohol; education and employment; social development and functioning; activities of daily living (e.g., self-care, living situation, nutrition, money management); and family structure and relationships.

* Servers as a coach for the team, including clinical leadership, team building, appropriate utilization and quality assurance, ensuring the involvement of all team members in all aspects of the team's work. Leads the daily organizational staff meetings.

* Meets individually and in groups at least monthly with practitioners to:

* Discuss treatment topics of interest.

* Discuss treatment plans of the individuals, and approve by signature and documentation any resulting updates.

* Directs the day-to-day clinical operations of the ACT team including scheduling staff work hours to assure appropriate coverage for the day.

* Provides direct clinical services including individual supportive therapy and psychotherapy to clients on an individual and family basis in the office and in community settings to teach behavioral symptom-management techniques to alleviate and manage symptoms not reduced with medication and to promote personal growth and development by assisting clients to adapt to and cope with internal and external stresses.

* Provide individual and group treatment in the office and in community settings in a stage-based treatment model that is non-confrontational, considers interactions of mental illness and substance abuse, and has client-determined goals.

* Coordinates with outside inpatient services to detoxify clients and established linkage to outpatient treatment, self-help programs (e.g., Alcoholics Anonymous, Narcotics, Anonymous), outpatient services, and residential facilities.

* Utilizes illness management and recovery strategies in collaboration with the Assertive Communitive Treatment Team, to provide ongoing evaluation, problem solving, side-by-side services, skill training and supervision, and environmental adaptations to assist recipients with activities of daily living to ensure maintenance of quality of life and successful community placement.

* In collaboration with the ACT Team, assist recipients to obtain and maintain community based employment consistent with the recipient's skills and abilities.

* Provide support and assistance to recipients in maintaining family and social relationships, reducing social isolation.

* In collaboration with the ACT Team, provides on-going assessment and intervention involving substance abuse when indicated.

* Participates in and is responsive to ACT Team quality assurance procedures, cooperates with ACT Team review and utilization procedures, including completion of recipient satisfaction surveys, and DACT Fidelity Standards review. Follows established procedures pertaining to maintenance of accurate and complete clinical records.

* Coordinates assistance for clients with other community professionals, also serves as a resource, advocate, and liaison.

* Facilitates Staff Team meetings and treatment planning review meetings per established meeting routine.

* Provides on-call intervention as delegated by the Director of Operations.

* Participates in the discharge planning process for a recipient being discharged from an Intensive Residential Services facility or inpatient hospital setting.

* Participates in transitioning recipients to appropriate level of care when discharged from ACT services.

* Available for evening and weekend work as required by ACT Team.

OTHER RESPONSIBILITIES:

* Ability to travel as position requires.

* Performs other work-related duties as assigned.

* Is sensitive and responsive to management needs such as data collection and program evaluation.

* While performing the duties of this job, the employee is regularly required to stand; use hands to finger, handle, or feel; and talk or hear.

* The employee frequently is required to travel to all SWMHC facilities in the catchment area, ambulate within and between departments; reach with hands and arms; stoop, kneel, crouch, or crawl; and taste or smell.

* The employee is occasionally required to sit and climb or balance.

* The employee must occasionally lift and/or move up to 40 pounds.

* Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.

REQUIRED COMMUNICATION/MATHEMATICAL/REASONING SKILLS:

* Must possess the required level of skills and abilities to read, write, speak, and apply mathematical concepts and sound reasoning to successfully perform the essential duties and responsibilities of this job.

* Must be able to perform basic operations on a computer within the Windows operating system.

* This position requires problem solving skills, critical thinking skills, and the ability to multi-task.
