Job Responsibilities
With our Agency's mission to support the recovery of people with mental illnesses, we provide a complete array of medical and support services for children, adults, and families throughout South Carolina.
* Services are provided in the community and in patients' homes in both Aiken and Barnwell counties.
Job Purpose
* Serves on the Assertive Community Treatment (ACT) team providing evidenced based services in an intensive non-residential treatment and rehabilitative services in accordance with the fidelity model. This is a single fixed point of responsibility for treatment, rehabilitation, and support needs for patients with Serious Mental Illness (SMI) who require higher levels of community care and have not been well supported in lower level of care options.
Job Function
* Provides direct services to ACT patients guiding them in the development of skills/strategics towards achieving and maintaining good mental health and facilitating training of independent living skills.
* Utilizes assertive engagement to engage beneficiaries in service.
* Assists patients with access to community services, resources and agencies and establishing support networks in the community.
* Utilizes the patient's natural supports.
* Completes screenings and assessments for referrals and assists with development and revisions of instructional continuity plans (ICP).
* Provides family psychoeducaton and works with beneficiary's natural supports.
* Leads the person-centered planning process for patients of the ACT program.
* Participates in clinical supervision, treatment team meetings, pursues professional development, and demonstrates effective use of social and interpersonal knowledge and skills and demonstrates an understanding and appreciation of individual and group differences and similarities.
* Effectively utilizes and maintains certification of evidence-based practices.
* Attends treatment team meetings daily and provides consultation regarding complex cases as appropriate.
* Completes documentation according to QA/SCDMH standards, Complies with Corporate Compliance Policy and Risk Management Plan, and Ensures timely completion of prior authorizations.
* Completes all annual required trainings to include Safety, MAPS, Cultural Affairs, and clinical trainings.
* Demonstrates adherence to CARF standards, SCDMH, and Center policies and Procedures.
* Performs other duties as needed.
Minimum and Additional Requirements
State Requirements:
* A Masters' degree in guidance, counseling or the social sciences or a related field.
Internal requirements:
* Licensed as an LISW-CP, LMFT, or LPC in the state of S.C.
* Master's or doctoral degree from a program that is primarily psychological in nature from an accredited university.
* One year of experience working with adults with Serious Mental Illness.

Preferred Qualifications
Agency Preferences:
* Practical therapeutic knowledge of principles, methods and counseling practices and procedures for the diagnosis and treatment of behavioral and mental disorders.
* Ability to listen and understand and communicate information and ideas so that others will understand.
* Knowledge of psychopathology, family dynamics, family and group psychotherapy.
* Ability to identify and effectively utilize resources available in the community to accomplish objectives.
* Ability to establish and maintain effective working relationships with individuals, families and representatives of other professional disciplines.
* Ability to present clear and comprehensive oral and written reports.
* Supervision provided by ACT team leader and Psychiatric Team Lead.
* Employee's activities are governed by Center and SCDMH policy and procedure.
* Ability to lift 5lbs and operate a state vehicle.
* Position is considered essential and may be called back during crises/disasters/emergencies.
* Position will be part of the Assertive Community Treatment Team.
* Requires flexible schedule and participation in after hours on-call rotation.
* Services are delivered in the community and patients' homes.
* Bilingual abilities in English and Spanish (or another language) are a plus.
