Job Summary: The primary responsibility of this position is to provide evaluation and assessment for mental health and substance use service need. The Mental Health Provider should possess a working knowledge of the spectrum of services and community resources that are available for this population of patients and their families. The Mental Health Provider works with the care team to provide assessment, service referral and continuity of care for assessed mental health and substance use needs. The Mental Health Provider serves as a resource and consultant to staff. The Mental Health Provider provides education to patients, and interdisciplinary staff regarding stress, coping and mental illness, treatment recommendations, referral resources, medication management resources, and referral placement. Responsibilities:
- Annual mental health assessments and evaluations for all enrolled patients.
- Annual substance use assessment and evaluations for all enrolled patients.
- Psychosocial assessments and reassessments as needed.
- Treatment referral services for mental health psychotherapy and psychiatric services.
- Treatment referral services for substance use treatment.
- Participation in semi-annual case conference meetings for all enrolled patients.
- Participation in the multi-disciplinary Quality Improvement program.
Minimum Qualifications: Master's Degree Master's degree in Social Work (LMSW, LCSW), Mental Health Counseling (MHC), or Marriage and Family Therapy (LMFT). Applicable NYS license require at time of appointment. Must successfully complete pre-employment and credentialing clearances.
Preferred Qualifications: Previous Mental Healthcare experience.
Work Days: Monday - Thursday, 8:00 am -4:30 pm Friday 8:00 am - 12:00 pm
