**Job Description :**
GENERAL OVERVIEW:
Maintains safety and structure by observing and recording patient information pertinent to the treatment plan and reports information to appropriate staff. Works under nursing supervision.
ESSENTIAL RESPONSIBILITIES:
+ Collaborates with the multi-disciplinary team to provide for an on-going assessment of changes in patient status and gathers pertinent patient care data through rounding and patient interaction using all available resources in the inpatient or outpatient setting. (60%)
+ Organizes patient care tasks according to priority and importance of tasks to be completed. (20%)
+ Obtains vital signs and height and weight measurements. Provides crisis intervention on the unit to maintain safety. Responds to facility Code events. (10%)
+ Reports any changes in physical, mental and emotional conditions to nursing staff and suggests modifications to the master treatment plan. (10%)
QUALIFICATIONS:
Minimum
+ Bachelor's degree or one to two years of relevant experience and/or education as determined by the company in lieu of bachelor's degree.
+ Child Abuse Clearance, CPR Certification, Criminal Background Clearance
+ Non-violent crisis intervention required within six months of hire
Preferred
+ 1 - 3 year of mental health or clinical experience
