Description:Mental Health Therapist-Behavioral Health OutpatientJOB PURPOSE:Provide Psychiatric and/or Chemical Dependency counseling services to the patients admitted to Piedmont Behavioral Health units.KEY RESPONSIBILITIES:1. Provides individual, group, and marital/ family therapy/ counseling in a variety of psychiatric settings.2. Performs Psychosocial Assessments.3. Provides treatment planning.4. Provides discharge planning including linkage to community resources, referral sources and post discharge care providers.5. Provides referrals for extended treatment or outpatient follow up.6. Conducts psychoeducational groups and classes for patients and families.7. Functions as a member of an interdisciplinary treatment team.8. Makes Post Discharge Wellness Check Calls on discharged patients.9. Ensures and maintains confidentiality and privacy of patient information.10. Maintains appropriate professional boundaries.11. Possesses excellent writing and verbal skills.12. Is knowledgeable of and follows all policies in daily job performance.13. Actively seeks ways to control costs without compromising patient safety, quality of care or the delivered.14. Attends in-service presentations and completes all mandatory education requirements.15. Regular, punctual and dependable attendance.16. Practices, develops and endorses customer service skills in relationships with internal and external customers.17. Basic computer skills. Good working knowledge of community resources for psychiatry, psychotherapy, and mutual support groups in local and surrounding area.
