  • Must possess a Masters Degree in Social Work, Counseling or Psychology
  • Current Masters level of Social Work or a Licensed Mental Health Practitioner in the State of Nebraska
  • Field Placement or one year experience working with Adult Psychiatric or Gero-Psychiatric patients is preferred
  • Experience with elderly clients in medical work/case management setting considered
  • Basic Life Support (BLS)
  • Advanced Cardiac Life Support (ACLS) preferred

SECURITY/ACCESS:
  • Will have access to the facility 24 hours a day.

Essential Job Duties and Responsibilities:
  • Conducts psychotherapy, family sessions, group therapy sessions, psycho educational group sessions and individual sessions.
    • Utilizes creative approaches to therapy and effectively engages patients in therapeutic process, displaying good theoretical knowledge of mental disorders and best practice/evidence based treatment techniques.
    • Encourages patients to express their feelings and discusses what is happening in their lives.
    • Helps them develop insight into their lives and relationships.
    • Guides patients in the development of skills and strategies.
    • Plans, organizes and leads structured programs of counseling, work study, recreation and social activities for clients.
    • Modifies treatment activities and approaches as needed in order to comply with changes in patients status.
    • Maintains professional boundaries.
  • Develops and implements treatment plans based on clinical experience and knowledge.
    • Collaborates with other staff members to perform clinical assessments and develops treatment plans.
    • Evaluates patients physical or mental conditions based on review of client information.
    • Monitors patients use of medications.
    • Meets with families, probation officers, policies and other interested parties in order to exchange necessary information during the treatment process.
  • Completes all required documents in as thorough and detailed manner within the established time frame.
    • Documents progress and communication notes in the required format on the day services were provided.
    • Documents the cause of a patients absence from groups on their assigned day and any (if applicable) therapist intervention.
    • Master Treatment Plan (MTP) developed within one week of admission with several short term goals immediately.
    • Psychosocial Assessment completed within three treatment days of admission.
    • MTP review completed every thirty days.
  • Demonstrates the ability to deliver limited patient care in a professional and caring manner.
    • Transfers patients in a safe manner, demonstrating proper uses of a gait belt and Sit and Stand Lift.
    • Assists with the patients personnel hygiene as the condition warrants, including toileting and hand washing/use of alcohol gel.
    • Takes vitals and collects routine specimens.
    • Serves as an advocate for the patient.
  • Refers patients, clients, or family members to community resources or specialists as necessary.
    • Participates in community outreach activities, including in-service presentation to relevant groups and agencies.
    • Fosters positive working relationships with area health care professionals and develops new referral sources via networking, speaking engagements, etc.
    • Plays an active role in the development and quarterly review of SWOT (strengths, weaknesses, opportunities and threats) and business plans.
  • Follows HIPAA regulations and ensures that the confidentiality of patients medical, personal, financial records, employee records, and organizational records is maintained.
  • Participates in FCH committees, quality assurance, performance and quality improvement initiatives, and activities which support the facility and department operations.
  • Complies with the state and federal regulatory requirements related to the performance of FCH operations and requirements of the FCH Compliance Program. Including but not limited to:
    • The Compliance Policy.
    • Completes all mandatory on-line training annually and as assigned.
    • All department and organization-wide policies.
    • Considers the impact of regulations such as Critical Access Hospital requirements, along with Federal and State of Nebraska regulations and discusses concerns appropriately to ensure compliance.
  • Presents self in a professional manner and enhances professional growth and development through participation in education programs, current literature, in-service meetings and workshops.
    • Seeks opportunities for continued growth and performance improvement.
    • Always maintains a professional appearance and manner.
    • Dependable and completes work in a timely manner.
