Mental Health Worker - Residence Per Diem
Job Summary:
You will coordinate program services, communicate observations and information about consumers to appropriate staff, provide verbal counseling and crisis intervention services, and help develop Individual Recovery Plans.
What You Will Do:
+ Prepare a complete written intake assessment which includes treatment plans, relevant, behaviorally-measurable goals that serve as indexes for evaluating treatment progress, and realistic time frames for achievement.
+ Perform ongoing assessment of consumers by exhibiting knowledge of age specific population, developmental level and diagnosis.
+ Assist the consumers to meet their needs by working on established goals developed in the multidisciplinary treatment plan.
+ Complete clinically based assessments at the end of a residential shift to ensure communication on the next shift or the following day.
+ Help with the acquisition of transportation and safely transport consumers daily.
What We Need to See:
+ A Bachelor's degree in behavioral science area from an accredited college or university
+ 1 years of verifiable related work or life experience
+ Experience in mental health, residential or partial care services desirable
+ A valid driver's license and must be insurable under Agency's automobile insurable policy
