The TM will work directly with the client in conjunction with the Mentor Coordinator to ensure mentoring components are being delivered in accordance with the mentee's Service Plan, TBRI principals, COA standards, and PCHAS mission of providing Christ-Centered care to children and families.Working Conditions: Works with clients in the schools and/or in the community.Essential Job Functions
  • Assist the Mentor Coordinator with the delivery of quality mentoring services, in accordance with the client's Service Plan, within the time frame targeted for those services, as well as communicate the client's progress and/or any needs the client may have.
  • Maintain accurate and timely records/case notes in CSWIS to ensure proper monitoring of service delivery, outcomes and plans for future reviews.
  • Participate in monthly supervisory sessions with your supervisor, monthly mentor meetings, and trainings as are scheduled. Utilize individual and group meetings as well as training opportunities for personal and professional growth.
  • Maintain both physical and public safety standards for PCHAS clients when they are under the supervision of a Therapeutic Mentor.
  • Assist with developing and maintaining a professional and collaborative relationship with the PCHAS team, school employees and various other community members, in an effort to provide the highest quality of services to TM clients.
  • Maintain mentoring caseloads according to PCHAS expectations per week: 1.) Full-time- 20 to 25 youth, minimum of 1 hour per week, with 10 minutes of documentation time per case note. 2.) Part-time- Contingent on hours per week, minimum of 1 hour per youth, per week and documentation time allowed accordingly.
  • As requested, provide Parent Aide services according to Children's Division expectations to assist parents with development of parenting and home management skills by teaching and modeling, with a goal of reaching an acceptable level of family functioning. (Farmington Service Center Mentors only)
RequirementsEducation- Minimum of High School diploma or GED/HISET. Some human services college courses, associate or bachelor's degree preferred. Upon employment, must pursue continuing education and/or in-service training as required for this job. Must have ability to develop and maintain therapeutic relationships with clients displaying behavioral or emotional problems. Must have the ability to maintain an appropriate balance between authoritative and supportive, with personal maturity being an extremely important attribute. Must be able to resolve problems, handle conflicts and make effective decisions under pressure. Must be flexible with working evenings and/or weekend hours to ensure clients are being seen weekly.Other Requirements: Must hold a valid driver's license, have and maintain a risk-free driving record. Must be willing to undergo initial and periodic child abuse/neglect, criminal history, sexual offender, and childcare/elder care disqualification list screenings . As requested, must submit fingerprints for review by the state of your employment and Federal Bureau of Investigation (FBI).
