Job Description:

The mission of the Mental Health Association is to promote mental health, with the integration of physical healthcare, to improve the care and treatment of individuals with mental illness, and to remove the stigma associated with mental health disorders and addictions.MHA is seeking a full time MICA Case Manager to join our Partial Care - Prospect House Team!Qualifications: Bachelor's degree in a human services field. Must have at least one year of related experience. Two years experience in providing co-occurring services; a CADC or LCADC is preferred. Must be proficient in the use of computers for the purpose of utilizing electronic medical records software. Valid driver's license and the ability to drive a van is required.Facilitation of MICA group required.Responsibilities: Facilitation of MICA group. Responsible for the daily implementation of the Partial Care Program, in collaboration with staff, supervisors and members, to provide partial care services to individuals with severe and persistent mental illness, in order to enhance community functioning.Plan and facilitate milieu activities, including therapy, coping, activity and psycho-educational groups to enhance social skills and promote relapse prevention and independent living.Collaborate with members and staff to accomplish psycho-social goals and tasks of the unit.Provide milieu management.Participate in unit trips including van drivingProvide case management services for members as assigned, including coordination of medical, psychiatric, financial, residential, social and legal needs of assigned caseload.Regular goal-setting with members.Crisis intervention, as neededAssist and advocate for members to obtain medical records, entitlements, especially Medicaid/Medicare and food stamps.Liaison with family, boarding home staff and local agencies which serve the elderly.Consult regularly with Prospect House program psychiatrists.Arrange transportation services and provide transportation as needed.Collaborate with the Program Director and Supervisor to develop the program, expand services, select supplies and materials and research new program methods and activities.Complete in-service training as required.Must meet all expected productivity levels in regard to direct service provision to consumers.Reports to: Program Director and Program CoordinatorFully paid medical coverage for eligible staff.Generous 401(k) match.Plentiful Paid Holidays, Vacation, Personal and Sick time.MHAEM is a proud equal opportunity/affirmative action employer committed to attracting, retaining, and maximizing the performance of a diverse and inclusive workforce.
