Job Title: Rehabilitation Therapist Job Specialty: Occupational Therapist Job Duration: 13 weeks Shift: Monday-Friday, Days Guaranteed Hours: 32 per week Experience: New graduates are welcome; Skilled Nursing Facility (SNF) experience preferred License: State License required Certifications: - 1-Step PPD or Quantiferon or Proof of Positive PPD and Chest X-Ray - Completion of a COVID test 72 hours prior to start date, with results issued 48 hours prior to start Must-Have: - Completed prescreen - Attached resume - License verification Job Description: - Ensure all pre-employment screening documents are complete and attached upon application submission. - Provides therapy services in a skilled nursing facility setting with a focus on improving the patient's ability to perform daily living and work-related activities. - Conducts patient assessments and develops individualized treatment plans. - Utilizes therapeutic techniques to enhance the patient's functional independence. - Collaborates with the healthcare team to provide comprehensive care. - Tracks and reports on patient progress. - Required background checks include County Criminal, Federal Criminal, Multi-State Criminal Search, Nationwide Sex Offender Search, HHS/OIG, GSA, Social Security Number Trace, and Minnesota State Background with fingerprints.
