Mobile Behavioral Health Rural Case Manager Consejo Counseling and Referral Service is looking to hire a full time Mobile Behavioral Health Rural Case Manager. Summary of Job Description: The Consejo Mobile Behavioral Health Rural Case Manager will travel to a variety of locations to offer additional access for our clients to our high quality, integrated behavioral healthcare services. Consejo's serves clients who are primarily low income, Latinx, BIPOC and who are from youth to adult. Many have been impacted locally and from their countries of origin by COVID, as well as other traumas. Our provider teams will focus on client need - emotional and substance disorders, domestic violence and sexual assault, and will coordinate services with Consejo's teams and other providers. Reporting to the Behavioral Health Clinical Supervisor the Behavioral Health Mobile Rural Case Manager will work in partnership with the client and mental health team in an effort to provide responsive, culturally sensitive, and comprehensive services to the clients of Consejo's Mental Health Department. The Mobile Behavioral Health Rural Case Manager will work with clients of different age ranges and backgrounds, persistent mental health concerns, and complex needs. The goal of the program is to provide responsive and comprehensive services to clients to improve their stability, wellness, and recovery. The Behavioral Health Mobile Rural Case Manager will assist in making referrals, coordinating care, and work with multiple systems including but not limited to jails, hospitals, vocational and housing programs, schools, legal systems, and medical providers. ESSENTIAL DUTIES AND RESPONSIBILITIES:
  • Understanding that transportation is often a barrier, Consejo provides services utilizing a decentralized model, meeting people where they are most comfortable, including at the clients' homes, in schools, spiritual setting, public libraries, and other community settings, in addition to office locations Renton and Bellevue.
  • The Behavioral Health Mobile Rural Case Manager will travel to a variety of locations to offer additional access for our clients to our high quality, integrated behavioral healthcare services.
  • Conduct behavioral health screening for MH/SUD clients in need of outpatient care.
  • Conduct community outreach, community presentation, participate in community and health fairs.
  • Conduct outreach efforts via radio programs, social media, Zoom, flyers, etc.
  • Outreach and collaborate with homeless programs, nursing homes, adult family homes, social service agencies, schools, primary care clinics and hospitals.
  • The mobile clinic is a secure, private space with the same behavioral health counseling services, substance use treatment services, care coordination, health education and information provided in our brick and mortar clinic locations.
  • The Behavioral Health Mobile Rural Case Manager will provide this decentralized model, is knowing, understanding and collaborating with the universe of service providers in the community, from first responders to other social service providers, physical and dental health providers, transportation, schools, Religious institutions, businesses, Boys and Girls clubs, Community Center, Local Public Libraries, and all the informal networks that communities maintain.
  • The Behavioral Health Mobile Rural Case Manager will provide services to all clients regardless their ability to pay.
  • Will maintain a caseload, size to be determined by BH Program Manager.
  • Provide a minimum of 12 direct client service contact per day.
  • Provide advocacy, outreach, information and referral services to mental health clients in need of case management services including wrap around services.
  • Coordinate services with the mental health therapists, other service providers and/or agencies to ensure optimal client care.
  • Assess crises and provide crisis interventions when necessary.
  • Lead or co-lead therapeutic groups as needed.
  • Establish liaisons to access community services, e.g. medical care, housing, food, etc.
  • Provide education, outreach and information in the community as needed.
  • Provide family support and support for others of significant importance to the client.
  • Serve as a liaison between the client and the multiple numbers of systems utilized by the client.
  • Advocate on behalf of the client to ensure that the client remains in school, home, and in the community when appropriate.
  • Connect the client and the family to appropriate resources in the community.
  • Reduce the client's law enforcement involvement, truancy, school suspension, juvenile detention/jail, psychiatric hospitalization, out-of-home placement, and homelessness.
  • Complete all required documentation in order to be 100% file compliant.
  • Attend mandatory clinical in-services/trainings, supervision, and regularly scheduled department/agency meetings.
  • Ensure behavioral health clients are enrolled in outpatient behavioral health services
  • Understanding of and ability to provide culturally appropriate and sensitive services.
  • Serve on committees, task forces and special assignments as directed
  • Other duties as assigned.
QUALIFICATIONS:
  • Bachelor's degree with major specialization in social, behavioral or human services or human related fields.
  • Relevant experience working with clients with behavioral health conditions, SUD disorder, comorbid conditions and co-occurring conditions.
  • Capacity to work well with children, youth, families, adults and older adults.
  • The employee is required to be able to safely operate a motor vehicle.
  • Possess and maintain a valid driver's license with an acceptable motor vehicle report and reliable, insured transportation.
  • The employee is required to take a driving training before driving the mobile clinic.
  • Proficient in speaking, reading and writing English and Spanish.
  • LICENSURE: Agency Affiliated Counselor Registration.
WORK SCHEDULE This full-time position works a typical schedule of 8 AM - 5 PM, Monday to Friday. SALARY$25.00 - $27.88/hr. depending on qualifications and experience. READY TO JOIN OUR TEAM?
