Collaborating with the Mobile Recovery Medical Care Coordinator and under the Mobile Program Manager's guidance, you will ensure the coordination and delivery of vital services to clients. The Integrated Care Coordinator will manage the care of specifically identified clients. Tasks include intake,discharge, billing and coordinating service delivery. They must understand and respect each individual's unique path to recovery and assist in creating a care plan reflective of that path. They must have a working knowledge of the drug and alcohol treatment system and a demonstrated commitmentto the recovery community. The role is to support others in recovery from a substance use disorder,serve as a point of contact to recovering individuals in order to help prevent relapse and promote long-term recovery. This position is a combination of direct client care and continuing development of anintegrated care model. This position will be reaching out to community partners to network as well ascomplete assessment for identified individuals. Collaborating with the Mobile Recovery Medical Care Coordinator and under the Mobile Program Manager's guidance, you will ensure the coordination and delivery of vital services to clients.ESSENTIAL FUNCTIONS- Coordinate all aspects of care for specifically identified individuals.- Completes assessments to determine level of care for identified individuals- Assist recovering people to identify their personal interests, goals, strengths and weaknessesregarding recovery.- Assist recovering persons in developing their own plan for advancing their recovery; anddocument that plan.- Works with Executive and Leadership teams to increase accessibility to integrated care model- Networks with community partners to increase access to integrated care model.- Serve as an advocate and ambassador to groups, activities and boards throughout thecommunity expanding pathways to recovery in the Grand Traverse area.- Serve on the Leadership Team and Recovery Center Steering Committee functioning as a liaisonbetween agency staff and Recovery Center staff.- Coordinate with Recovery Center Manager and lead recovery planning facilitating the transitionfrom a professionally directed service plan to a self-directed Recovery Plan; to transition fromprofessionally assisted recovery initiation to personally directed, community supported recoverymaintenance.- Actively identify and support linkages to community resources that support the recoveringperson's goals and interests involving the person, agency staff and other relevant stakeholders.Maintain accurate and timely records of all aspects of care to ensure appropriate billing andfinancial reimbursements.- Identify barriers, internal and external, to full participation in community resources and developstrategies to overcomeQUALIFACTIONS:Peer Coaching Certification or community health worker certification consideredBachelors Degree in Human Services PreferredREQUIRED TRAININGS/CERTIFICATIONS:CPR/First Aid Certification upon hire Valid Michigan Chauffeur License upon hire MCBAPCertified or MCBAP Development Plan upon hire. Recovery Coach certified or train upon hire.Adherence to ATS training requirements.SKILLS NEEDED/EXPERIENCE:- Must possess excellent written and verbal communication skills.- Must possess excellent decision making and critical thinking skills and ability to function on amulti- disciplinary team with an ability to think outside the box and consider multipleperspectives.- Knowledge of recovery and addiction treatment models.- Strong crisis intervention skills to effectively deescalate and problem-solve to promote safetyand stability.- Knowledge and demonstration of maintaining appropriate boundaries in staff, client andcommunity relationships.This job description is intended to indicate the kinds of tasks and levels of work that will be requiredof this position.
