About the Job

Mobile Therapist provide outreach to individuals, within their home setting, who have serious and persistent mental illness. A key component of this position is to work with individuals to access, link, coordinate and monitor needed services as well as provide support and advocacy. The primary goal of these services is to maintain the individual within the least restrictive setting to achieve a level of independence and mental health stability while remaining linked to all necessary service providers. Documentation is an important component of this role assuring the writing is person centered and demonstrates work toward goals as based on the service/treatment plan.

Responsibilities

  • Preparing and maintaining timely, accurate, and complete records
  • Participating as a team member to improve the overall effectiveness of one's work group and to achieve group goals
  • Ensures interventions and supports are delivered in a way that incorporates consistency, flexibility, and responsiveness to changing client needs.
  • Involves clients in identifying and coordinating treatment issues.
  • Empowers clients through therapeutic rapport, advocacy, and skill building.
  • Exhibits proficiency with benefit and entitlement options to maximize treatment planning opportunities.
  • Coordinates emergency psychiatric services within a timely manner, including documenting interventions, alternatives considered, and response of client/client family, and supporting agencies.

Qualifications

  • Master's degree in counseling, social work, clinical psychology or closely related discipline required.
  • Licensure preferred (LPC,LCSW) but not required. (Individual and group clinical supervision is offered to applicants working toward licensure.)
  • A commitment to community based mental health services.
  • Thorough understanding of the biopsychosocial aspects of mental illnesses and counseling techniques, including use of evidenced-based practices.
  • Familiar with and motivated to use a strength-based, person-centered approach to treatment.
  • State driver's license and record in good standing.
  • Computer literacy.
  • Working effectively and cooperatively with co-workers (e.g., peers, management). Showing appreciation and respect for co-workers' values, ideas, and perspectives. Demonstrating a willingness to participate as a member of a team.
  • Travel required: all sessions will occur in residences/community
  • Valid driver's license and record in good standing
