Are you a caregiver or parent of a child who is disabled or with mental health issues.Job Description:Family Partner will work as a team with a Clinician to provide child and family driven mobile outreach services as a means of crisis intervention, hospital diversion, and skill building to support healthy family functioning and goal setting. Short term mobile response team will work with children and family for a minimum of initial meeting and up to 8 weeks as driven by the family.In this role you will:Family Partner's services include an array of formal and informal services and supports provided to families raising a child who is experiencing social, emotional, developmental and/or behavioral challenges in their home, school, and/or community. Activities can consist of engaging the parent/caregiver in activities in the home and community that are designed to address one or more goals on the waiver participants service plan; assisting parent/caregiver in meeting the needs of the youth through educating, supporting, coaching, modeling and guiding; teaching parent/caregiver how to network/link to community resources and treatment providers; teaching parent/caregiver how to advocate for services and resources to meet the youth's needs; and guiding and support linkage to individual, peer/parent support, and self-help groups for parent/caregiver.Family Partner have first-hand knowledge of the services and supports available in the community.Family Partners offer resources including, but not limited to:Information on community resourcesAssisting families in connecting to community resources and natural supportsStrengthen and support the care-giving efforts of families with special emphasis on needs such as: emotional, physical health, parenting, and family interactionEmpower families to make informed choices regarding the nature of supports for themselves and their child by providing an understanding of what resources, services and supports are available and how to access them in their communitiesDevelop a family's capacity to actively participate in all decisions about services and supports for themselves and their childDevelop a family's capacity to enhance and improve the overall health and well-being of their child and familyWork with the family and their provider team to promote effective collaboration and communicationStrengthen and develop a family's skill and feeling of self-efficacy so they can effectively advocate for their child, work collaboratively with services providers and do so with increasing independence over timeAdvocate with the family to access supports, services, and activitiesSupporting the efforts of the families in caring for and strengthening their children's mental and physical health, development and well-being of their children.Qualifications:The ideal candidate will:Be able to complete Mandated Reporting self-directed on line trainingHave experience working with children (preference given to those with experience working with children with special needs)Be a parent or caregiver of a child with a history of emotional and/or behavioral problemsBe able to complete the Individualized Care ModelBe able to complete and obtain their PEP CertificationComplete safety in the home community trainingComplete OMH online Foundations trainin gEEO, AAE M/F/D/VIND2024Share: mailApply Now
