Preferred experience includes Acute Care, Housing Experience, Substance Use Disorder, Grief and Bereavement, Cultural Responsiveness, Crisis Intervention. License: Must sit for or obtain LCSW within 90 days Certifications: Basic Life Support (BLS) Must-Have: - Master's degree in Social Work- Licensed at the Master's level Job Description: - Work in an inpatient setting in the Surgical ICU and Intermediate Care units.- Collaborate with nurse case managers to assist with discharge planning.- Conduct biopsychosocial assessments to evaluate social determinants of health.- Provide emotional and resource support through strength-based and trauma-informed interventions.- Collaborate with multi-disciplinary teams to address how illness or injury affects patients, families, employment, and living situations.- Advocate, educate, and empower patients and families to promote health and reduce health inequities.- Handle a daily caseload of 15-20 patient referrals for social work consultations.- Ensure access to quality healthcare and work on reducing barriers to discharge.Please note that successful candidates will have a strong commitment to improving patient care and outcomes in a dynamic healthcare environment.
