Job Title: Registered Nurse (RN) - Hospice & Home Health Job Specialty: Hospice RN, Home Health RN Job Duration: 13 months Shift: Monday to Friday, 8 AM - 5 PM with on-call responsibilities Guaranteed Hours: 40 hours per week Experience: Minimum of 2 years of recent acute care, home care, or hospice experience License: Current RN license in the State of Montana Certifications: BSN preferred Must-Have: - Valid driver's license- Appropriate auto insurance- Reliable transportation Job Description: - Provide skilled nursing and coordinate care for patients and family members in a residential setting through a team approach and based on systematic evaluation as authorized by a physician.- Observe, assess, evaluate, and plan interventions for patients' physical and psychosocial statuses, including changes in condition and reactions to treatments or medications.- Oversee care provided by Home Health Aides and CNAs to assigned patients and evaluate compliance with plan of care.- Ensure continuity of care through effective communication, information exchange, and case conferences with other disciplines, community health, support, and social agencies.- Deliver quality care and appropriate symptom management for positive clinical outcomes and promote optimal health and quality of life through teaching, counseling, interventions, and referrals.- Maintain accurate, current, and complete patient records in accordance with agency standards and OASIS requirements, as well as knowledge of current reimbursement rules and regulations.- Participate in team scheduling coordination and on-call rotation schedule by assessing and evaluating patient needs via telephone and advising or making home visits as required.- Engage in quality improvement activities and attend in-service training as assigned.- Perform other duties as assigned by the supervisor.
