DescriptionSalary: $52,000/yearPosition Summary: This professional position serves as the centralized point of contact for all Multidisciplinary Staffings which include the child, family, natural and community supports, and professionals who collaborate to empower, motivate, and strengthen the family and develop an individualized plan best suited for their needs. This position will ensure staffing's occur within standards outlined in SB 80, established time frames, including all relevant team members, allows for all members to have voice and input, address child safety, permanency, and well-being. In addition, this position will act as support for children and youth requiring a staffing through Every Student Succeeds Act (ESSA) to ensure educational stability for children in Out of Home Care.Technical/Functional Expectations:
Multidisciplinary Team (MDT) Point of Contact-Essential Function: This position is responsible for bringing together a diverse group of individuals for the purpose of allowing better engagement with families and a share commitment and accountability. This position is responsible for scheduling, coordinating, and facilitating MDTs for all initial placements, changes in physical custody, changes in educational placement, placement decisions involving siblings, and other complex important decisions related to the child. Multidisciplinary Team (MDT) Assessment - Essential Function: Ensure the MDT conduct an assessment before formulating a decision. In cases where there is not a unanimous consensus the Department will make a best interest determination and report to the court within established time frames. Transition Plans - Essential Function: Coordinate, track and ensure completion of transition plans for all: LOHC placements, siblings, education within the required time frames. Ensure that all transition plans are documented and uploaded in FSFN. Separated Siblings: Coordinate, schedule and review all separate siblings minimally every quarter to assess for barriers to placement together and ensure sibling visitation is occurring, and contact is being maintained in other ways. On Call Rotation - Essential Function: Participate in the first tier on call rotation to provide timely service delivery. Customer Service - Essential Function: Work with clients and families to trouble shoot and assist with problem solving. To ensure client satisfaction. Ethics: Adhere to and model principles and values of Agency and System of Care by being strength-based, solution-focused, maintains highest level of integrity and ethical standards and work collaboratively with employees, partners, stakeholders, and clients in all interactions.Business Experience: Valid Driver's License-Essential: This position requires a valid driver's license. 5 years' experience in child welfare or human svc experience: This position requires at least 5 years of experience in the intake of children in the child welfare system or human services field. Computer skills required: This position will be required to work in Microsoft Word, Excel, and Access. Excellent Analytical and Problem-Solving Skills: This function requires the ability to independently assess, evaluate, and make recommendations regarding placement and services for children in licensed out of home care. Facilitation of meetings-Essential: Experience in coordinating and leading team meetings. Strong interpersonal, presentation, and organizational skill: This is required to effectively partner with various stakeholders and agency staff. Strength Based Family Practice Orientation: This is required to effectively support the system of care and values. Strong communication skills: This is essential to develop written summaries and recommendations for distribution within the system of care. Intermediate to advance level skills in MS Office to include Word, Excel and PowerPoint and familiarity and comfort using databases to populate and evaluate data, make case notes, and pull data.Educational and Experience Requirements: BA/BS required in human services related field. 2 to 5 years related experience in child welfare required.
