JOB DESCRIPTION
POSITION TITLE: Navigator
FLSA CLASSIFICATION: Non-Exempt
REPORTS TO: Director or Prevention and Compeer (MHASCK)

POSITION OVERVIEW: The Navigator will work in Nursing Homes for Mental Health across the State of Kansas with residents who have a desire to transition into the community. Utilizing the person-centered approach, the Navigator is responsible for the coordination of care, collaborating with NFMH residents, guardians, NFMH Liaisons, CMHCs, MCOs, and KDADS to provide wraparound services for clients who express interest in existing an NFMH. The Navigator is also responsible for collecting participant data, case management, discharge planning, and advocacy. The position requires frequent travel and attending program-related meetings as needed/requested.
ESSENTIAL POSITION RESPONSIBILITIES:
1. Provides services to meet program goals, maintaining a caseload of approximately 30 clients at a time.
2. Meets deadlines and ensures accuracy of various reports/paperwork, mileage, and timecard entries.
3. Maintains accurate and timely documentation of service provision. Completes case notes in a manner that individualizes each note, reflecting appropriate interventions and progress towards goals.
4. Schedules and participates in the development of a Person Centered Care Plan with the client, family/caregiver, and CMHC NFMH Liaison utilizing the Preadmission Screening and Resident Review (PASRR).
5. Provides periodic review of Care and WRAP plans to monitor the client's progress toward goals.
6. Develops a crisis plan with client using the WRAP book and WRAP card.
7. Monitors status of clients and provides level of personal and other supports consistent with client status. Notifies CMHC NFMH Liaison and Director of change in client status.
8. Coordinates other services required to meet identified goals based on the needs of the clients and congruent with the client's level of functioning.
9. Coordinates care and facilitates discharge plan work with client, MCO, CMHC, NFMH, KDADS, and caregiver/guardian.
10. Attends all department/individual meetings, agency meetings, and trainings as required.

1. Maintains acceptable overall attendance record. Ensures appropriate notification to supervisor for absences and ensures that work is covered. Flexible in work schedule when needed.

POSITION REQUIREMENTS: Applicants must possess a Bachelor's degree in a behavioral health field and have at least 3 years experience. Preference will be given to those who have experience in working with individuals who have a dual diagnosis of substance use and mental illness; or those who have experience working with NFMHs and/or CMHCs. Applicants must have a valid Kansas driver's license and consistent to a background check. Frequent out of town travel is required.
