We are currently looking for a Full Time Navigator to join our Medical Family Therapy Team A team located at the Chickasaw Children's Village working 11 am to 8 pm.Summary:Responsible and accountable for direct and indirect patient care across the care continuum, with an emphasis on outpatient care, for designated populations.Responsibilities:* Coordinates and facilitates initial consultation and screenings in order to assess service needs* Accurately documents all navigator proceedings through appropriate recordkeeping in the electronic health records system (RPMS/EHR)* Develops and maintains cooperative referral relationships; facilitates and tracks referrals* Identifies, coordinates, and connects individuals to available resources* Maintains an up-to-date log for all patient callbacks, including call dates, outcomes/results, next planned contact, and related information needed to accurately track and report statistics of patient contacts* Independently, or with assistance of qualified team members, utilizes and teaches problem solving techniques to individuals and/or groups* Serves as the system navigator for divisions' shared patients/families and is directly accessible as the point of contact to address their questions and concerns* Assists patients/families in resolving potential issues related to health care and financial or social barriers e.g., request interpreters, transportation services* Provides immediate crisis diversion* Attends both mandatory in-service trainings and related conferences and shares content of educational programs attended with team* Recovery Resource Services program only: Navigator is responsible for coordinating all available and necessary services for drug court participants including, but not limited to, monitoring each participant's progress throughout the drug court process, working closely with the court system and law enforcement, attending drug court staffing, and other duties specific to the program* Records care given and reports patient care observations* Provides first class customer service - courteous and helpful to all internal and external customer types- Answers questions in a courteous and professional manner- Partners with internal and external services as needed to ensure appropriate referral is made- Greets and engages with a smile- Determines and meets needs- Seeks positive experience and outcomes* Completes all training and development requirements and participates in continuing education opportunities* Works in a collaborative and supportive manner to support the productivity and safety of the work area, team members and customers of all types- Shares responsibilities for maintaining adequate supplies and equipment- Maintains general upkeep of work area and maintains a safe environment- Observes and addresses potential environmental hazards and risks- Maintains and promotes professionalism- Punctual and consistent in meeting start time, work schedule, attendance and other assigned work time metrics- Adheres to Chickasaw Nation dress code standards- May provide break coverage for others as assigned- May augment staff in other areas as needed based on staffing and needs ratio in regularly assigned unit* Maintains knowledge of and follows all laws, rules, regulations, policies and SOPs for assigned area, e.g. MICS, TICS, hospital standards, health codes, food sanitation, accounting principles, etc.* Performs all other duties as assignedCompetencies:Essential:* Follow instructions, both written and oral * Adhere to all scheduling and attendance requirements* Maintains strict confidentiality in all matters* Good driving record and be insurable by the Chickasaw Nation* Ability to work independently* Honesty, truthfulness, reliability, accountability* Read, understand and apply regulations and policies* Understand Native American culture and tribal programs* Ability to train individuals or small groups on specific tasks or procedures and engage with different learning styles* Communicates verbally with team, departments, guests and management* Communicates clinical information to patient in non-emergent setting* Keeps written recordsSkills:Essential:* Basic Computer skillsExperience:Essential:* 1 year directly related experienceEducation:Essential:* Bachelors Degree in directly related fieldCredentials:Essential:* Pass general background check* Standard Driver's LicenseWorking Conditions:Essential:* Fast-paced* Mental health treatment facility* Mix of office/non-office settings* Team-oriented* May be required to assist individuals who are emotionally disturbed* High pressure* Hostile, agitated or under-influenced persons* Interacts with patients in difficult and/or emergency situations* Interacts with clinical, support, technical, professional or management staff in a variety of settings and situations* Pass pre-employment drug test* Sometimes bend, kneel, stoop, push, reach* Use computer up to 50% of shift* Sometimes lift and or carry up to 25 lbs* Light to non-strenuous work* Sitting up to 50% of shift* Walking up to 25% of shift* Subject to traveling and working at other locations or in other environments* Travels up to 25%
