$23.07 - $36.91 Hourly$47,989 - $76,782 AnnuallyThis position is a Pay Band 65Posting DetailsAging and Long-Term Services Department's (ALTSD) mission is the belief that New Mxico's older adults and adults with disabilities have the right to remain active participants in their communities, to age with respect and dignity, to be protected from abuse, neglect, and exploitation and to have equal access to health care. The Long-Term Care programs at ALTSD administer community-based programs to allow older adults to thrive in their homes and communities to reduce nursing home placements and emergency department visits. Why does the job exist?Under the Direction of the Division/Program Director, the New MexiCare Program Coordinator will: *Oversee program rules and eligibility for New MexiCare*Provide technical assistance and support to participants, caregivers, and contractors*Resolve complex client eligibility and payment issues*Conduct in-home visits and complete assessments; *Track and report for KPI's and Performance Measures , accurately and timely, as established by the agency How does it get done?At the direction of the Division/Program Director, the New MexiCare Program Coordinator is responsible for: *New MexiCare program oversight and support to include eligibility determinations, financial assessments, and presenting on New MexiCare programs.*Tracking and monitoring of participants and caregivers cases, to include eligibility status, budget status, service planning, and follow up as needed. *Provide technical assistance and support to participants, caregivers, and contractors*Resolve complex client eligibility and payment issues*Conduct in-home visits and complete assessments*Track and report for KPI's and Performance Measures, accurately and timely, as established by the agency Who are the customers?ALTSD clients, other agency staff, contractors, home care agencies, and community organizations. Ideal CandidateThe Ideal Candidate is someone who can multi-task, work autonomously, and provide outstanding customer service to a wide range of individuals. The person must demonstrate strong critical thinking, case management, working with diverse individuals, working in the field in clients' homes that may have health or safety concerns, conducting assessments, completing documentation, and ensuring all reporting is accurate and timely. This position requires travel and overnight stay.* Experience working with high volume case loads which offer guidance and direction to clients.*Experience working in home and community based programs*Knowledge of Community Based Programs*Experience working with Word, Excel, and PowerPoint*Experience in writing, reviewing and implementing policies and procedures Minimum QualificationBachelor's degree in Social Work, Psychology, Guidance, and Counseling, Education,Sociology, Criminal Justice, Criminology and/or Family Studies/Services and two (2) years of any combination of experience including working with communities, working on health or socialservice related matters, social work/case management experience, behavioral health, and/or health care. Any combination of education from an accredited college or university in a relatedfield and/or direct experience in this occupation totaling six (6) years may substitute for the required education and experience. Substitution TableThese combinations of education and experience qualify you for the position: Education Experience Education Experience 1 High School Diploma or Equivalent AND 6 years of experience OR High School Diploma or Equivalent AND 6 years of experience 2 Associate's degree in the field(s) specified in the minimum qualification AND 4 years of experience Associate's degree or higher in any field AND 6 years of experience 3 Bachelor's degree in the field(s) specified in the minimum qualification AND 2 years of experience 4 Master's degree or higher in the field(s) specified in the minimum qualification AND 0 years of experience •Education and years of experience must be related to the purpose of the position. •If Minimum Qualification requires a specific number of semester hours in a field (e.g. 6 semester hours in Accounting), applicants MUST have those semester hours in order to meet the minimum qualifications. No substitutions apply for semester hours.Employment RequirementsMust possess and maintain a valid Driver's License. Must possess and maintain a current Defensive Driving Course Certificate from the State of New Mexico or must pass and receive Defensive Driving Course Certification within 90 days of employment. Must pass State and Federal Criminal background check. Working ConditionsWork is performed in an office setting and community setting with extensive use of personal computer, telephone, fax, copier, and other related office equipment.
