Description:The Night Shift LVN reports to the Director of Patient Care Services and is responsible for providing direct patient care under the supervision of the Director or designee. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.Job Responsibilities1. Triages phone calls from patients/families.2. Makes home visits as staffing issues dictate. Works cooperatively with patients, families, nursing staff, physicians, consultant personnel, and ancillary service providers.3. Reports DPCS/Clinical Manager and day shift nurses with patient updates from night shift with daily reports and EMR charting.4. Works cooperatively with patients, families, nursing staff, physicians, consultant personnel, and ancillary service providers.Professional and Technical Development1. Identify personal education needs and seek formal programs or participate in educational activities or seeks other avenues to meet these needs.2. Develop a work style, which demonstrates teamwork, constructive time management as well as effective conflict resolution and problem solving.3. Participate in the evaluation(s) of own performance by identifying strengths and weaknesses and by developing a plan to improve areas of weakness.4. The incumbent may, from time to time, be requested to perform job-related tasks other than those stated.Requirements:Qualifications1. California LVN license required, and CPR certified.2. Minimum of one year hospice experience desired; two years SNF/home care experience preferred.3. Must be knowledgeable, at minimum, about Hospice Medicare Conditions of Participation and California Hospice Licensing Standards.4. Excellent communication skills, both verbal and written, are required.5. Ability to work effectively with community physicians and other health care providers (all disciplines) toward specified goals is required.6. Must be self-motivating individual who can work autonomously, set own priorities, organize work, take initiative and make independent decisions relating to job tasks.7. Knowledge and experience using electronic medical software systems (EMR), Allscripts a plus.8. Must be computer literate in Microsoft Word. Power Point software is preferred.7. Ability to handle frequent interruptions and to complete different duties in a tight time frame.9. Ability to successfully meet agency background screening requirements.Job Specifications1. Ability to safely lift medical equipment (e.g. a wheelchair) weighing up to 35 pounds and to assist family/caregivers with transferring and ambulating patients as safety requires; considerable walking and standing may be required.2. Must have visual acuity and hearing to perform required nursing tasks and duties.3. Ability and willingness to flex or add work hours to meet scheduled shifts and holidays is required.4. Ability and willingness to maintain and drive a personal vehicle. A valid California driver's license and state-required automobile insurance are required.5.
