Position Summary: Our client is seeking a dedicated Case Manager to provide essential support and advocacy for individuals served by their programs. The Case Manager will manage a caseload of 15 to 25 clients, assisting them with various needs such as securing housing, managing daily living tasks, and accessing necessary services. This role is crucial in ensuring clients receive the support and resources they need to improve their quality of life.
Key Responsibilities:
- Client Management: Oversee a caseload of 15 to 25 clients, offering comprehensive support and guidance.
- Service Coordination: Help clients access housing, obtain identification documents, and navigate other essential services.
- Weekly Check-Ins: Conduct regular check-ins with clients to monitor progress, address issues, and provide ongoing support.
- Appointment Assistance: Use a company vehicle to transport clients to appointments and other necessary visits, ensuring timely and reliable transportation.
- Documentation: Maintain accurate and detailed records of client interactions, progress, and status changes.
- Collaboration: Work with team members, community organizations, and service providers to deliver holistic care to clients.
- Compliance: Adhere to organizational policies and successfully complete federal background checks and drug screening prior to employment.
Qualifications:
- Passion and Compassion: A genuine commitment to supporting individuals and a strong desire to make a positive impact.
- Experience: Previous experience in social services, case management, or related fields is preferred but not required.
- Skills: Strong organizational, communication, and problem-solving abilities. Capable of handling sensitive situations with empathy and professionalism.
- Driving Record: A valid driver's license with a clean driving record (less than 3 points).
- Background Check: Ability to pass a federal background/fingerprint check and drug screening.
