Key Job Responsibilities
  • Examine residents and deliver quality care in varied settings to include subacute, long term, rehab and outpatient care
  • Order diagnostic workups and tests and timely completion of medical records and discharge summaries in a timely manner
  • Assist in resident/patient/family education and re-examine, re-evaluate and modify care plans as necessary, including maintaining proper documentation in resident charts
  • Prescribe medication as necessary
  • Review all diagnostic testing results and make recommendations to findings
  • Ensure Encounter forms are completed accurately and in a timely manner
  • Educate and supervisor rotating medical and physician assistant students
  • Participate in on call monthly/weekend medical coverage
  • Maintain continual medical education credits
Position Qualifications
  • Two (2) or more years of experience in a sub-acute or long term care facility administering medical care to residents
  • Graduate of an accredited physician assistant or nurse practitioner program
  • Clean and active license as an NP or PA New York State, with prescriptive privileges
  • Proficient in SigmaCare or another EMR (Electronic Medical Record) software
  • Prior experience with the geriatric population a plus
  • Fully vaccinated against COVID-19
