+ SCRMC LTC-Certified Nursing Assistant JCRH CCNC DESCRIPTION Job Title: 254 255 695 Certified Nursing Assistant Department: 7800 7820 LTC General Responsibilities: -Verbalizes role in Emergency Operation Plan including response to fire alarm (including responding to drills) and can locate fire pull alarm, emergency exits, fire extinguishers and Emergency Operations Manual (EOP). -Able to verbalize role regarding Resident's Rights, Resident Abuse, Neglect, Exploitation and can locate Abuse Information Manual. -Understands role regarding HIPAA and practices resident confidentiality. -Able to verbalize role regarding Resident Elopement and can locate Elopement Information Manual. Responds to door alarms and drills. -Able to locate and proficiently use job specific equipment and supplies. -Participates in Service Excellence and QAPI process. -Proficient with computer programs used for job specific tasks. Completes required in-service training in a timely manner. -Communicates appropriately with residents, staff, visitors and others. Uses proper telephone etiquette. -Verbalizes role and understands how to file an incident report or EOR. -Able to verbalize role regarding Restraint Use and Resident Fall Management. Can locate the Falls Information Manual if needed. -Informed and Compliant regarding current phone number on file, breaks, assignments, attendance, call-in log, scheduling requests, and staffing requirements. -Adheres to infection control policies. Practices proper hand hygiene. -Assists in maintaining a clean and safe environment. Participates in cleaning duties as identified on the Cleaning Log and as directed by the nurse on duty. -Demonstrates knowledge of the survey process (CMS & DNV) Documentation: -Collects and records I&O as directed by the nurse. -Collects and records intake for Calorie Count. -Completes all Scheduled Tasks and documents in a timely manner. -Communicates Resident Problems/Needs. Uses Problem Report form as needed. Resident Care: -Ensures that resident's personal care needs are met (hygiene, oral care, hair care, skin care, dressed appropriately, groomed, nail care etc.) -Serves meal trays appropriately, snacks, assists with meals, provides fresh water or fluids, thickened liquids. (Water pitchers changed daily on evening shift.) -Ensures bed linens are always clean and fresh (changed on bath days and prn.) -Ensures resident's rooms are neat and orderly as daily care is provided. (Including use and storage of personal care items.) -Performs catheter care or peri-care appropriately. -Performs proper peg site care. -Proficient in Whirlpool use. Has viewed the training video. -Proficient in taking vital signs. -Proficient in using mechanical lift. Has viewed the training video. -Follows resident plan of care including bath/shower schedule. -Answers call lights promptly.
