Job Description: Job Overview: We are hiring a Registered Nurse Assessor for the New York Independent Assessor Program (NYIAP). In this role, you'll conduct health assessments to determine eligibility for long-term community-based services for Medicaid recipients (ages 19+). Assessments are conducted via telehealth or in-person at the consumer's home, with a flexible schedule based on consumer-driven appointments. Key Responsibilities:Conduct health assessments (telehealth or in-home) for Medicaid-eligible individuals.Review and summarize medical documentation to determine care needs and eligibility.Enter assessment data into the UAS-NY online system and document all appointments.Collaborate with the clinical team to ensure accurate and high-quality assessments.Stay updated on changes in regulations and assessment guidelines.Travel within your assigned region to meet with consumers in their homes.
