DescriptionBring a unique blend of clinical expertise and administrative acumen to the role by reviewing med records and meeting with patients and team members on a weekly basis. With a proven track record of optimizing operational workflows, ensuring compliance with healthcare regulations, and enhancing patient satisfaction, you are dedicated to improving both the efficiency of healthcare services and the quality of patient outcomes in the Behavioral Health Care Partners program.What You Will be Doing as a Behavioral Health Nurse...* Collaborate with treatment team (including collaterals) to prepare necessary information to complete medication reconciliation, including but not limited to: obtaining medication lists, discharge summaries and other documentations as necessary, entering that information into the electronic health record.* Communicate directly with enrollees via telephonic, electronic ways and/or face-to-face and document all activities as required in the EHR system.* Perform comprehensive assessment reviews, and treatment plan / care plan reviews and sign off on them as part of the MDT.* Request medical records from PCP, higher level of care and other collaterals, and follow up on the requests to ensure that the MDT gets the record.* Adjust medication list when needed.Schedule: Flexible, hours will be 16-20 hours per weekImportant Health and Wellness Benefits which Begin First Day of Employment* Blue Cross and Blue Shield Health and Dental Insurance* Eye-Med Vision Benefits* Employer Paid Life and Long-Term Disability Insurance* Medical Flexible Spending Account and Dependent Care Account* Employee Assistance ProgramGenerous Paid Time Off* 35 Days Paid Time Off (15 Vacation Days, 12 Holidays including Juneteenth and 8 Sick Days)Additional Benefits* Retirement Plan 403(b) (employer match after the first year of employment)* Opportunities for Student Loan Forgiveness* Mileage reimbursement* Extensive Training Program including CEU's.* Supervision for Licensure and Specializations* Opportunities for Certification in Evidence Based PracticesRequirements* Valid Massachusetts Registered Nursing License.* +2 years of experience working with mental health and substance use disorders.* Proficiency in communication technologies (email, writing skills, cell phone, etc.).* Has knowledge of mental disorders (including SUD, COD, SMI, SED) and physical illnesses, and has knowledge of procedures and treatment, related medications, acuity vs. chronicity, effect on normal growth and development.* Has demonstrated competency in age specific behaviors, cultures, effects of illness as well as family member's illness on age-related development.* Highly organized with the ability to keep accurate notes and records.* Experience with EMR systems and experience working with multidisciplinary team (MDT).* Has reliable transportation and valid MA driving license with acceptable driving record and current proof of insurance.* Experience with transitions of care and familiarity with system of care.* Has knowledge of resources for SDOH, and able to advocate / negotiate systems for and with enrollees.* Advanced crisis intervention/treatment/management skills preferred.* Has strong organizational and communication skills.* Acceptable CORI and SORI background record check as required by programBay State Community Services is an equal opportunity employer.
