Nurse Care Manager Job Code:2024-30-15-007 Location:Port Orchard Department:Behavioral Health Full Time/Part TimeRegular Full Time Job Responsibilities: Job Summary The Nurse Care Manager is responsible to educate, assess, monitor, and assist patients during induction, stabilization and maintenance phases during treatment for opioid use disorder (OUD). Essential Duties and Responsibilities * Work with Medication Assisted Treatment (MAT) team to support the OUD patients * Work closely with providers to facilitate treatment plan * Support providers in all phases of treatment, including injection visits, random urine toxicology screens, routine labs, pill counts, call-backs/follow-ups and facilitate medication refills * Under the direction of a provider acting under the scope of their license to administer drugs, medications, treatments, test, injections and inoculation. * Continually assess patient's need for ancillary services and ensure care is integrated across all programs including counseling, mental health, substance use and primary care needs * Provide follow up patient care in person and via telephone when needed * Collaborate, communicate, and meet with MAT team as needed * Follow State and Federal guidelines in providing care to opioid dependent patients in collaboration with licensed, prescribing physicians and pharmacies * Maintain appropriate documentation; collect pertinent data using appropriate assessment techniques * Consistently use clear, concise, and effective written/oral communication and comply with departmental policies when sharing/documenting relevant patient care data * Familiar with using electronic health record (Ex. Athena) * Hours may include weekends * Other duties as assigned Supervisory Responsibilities This position has no direct reports. Pay Range: $20.32 - $30.23/hour DOE Qualifications * Active LPN or MA license in Washington State (required) * Active CPR Certification (required) * Valid driver's license (required) * One to two years of customer service experience (preferred) Skills Must have excellent communication skills. Ability to work independently and as part of a team; to receive and respond well to feedback. Possess strong interpersonal and problem-solving skills; time-management and organizational skills, including being punctual, responsive, and efficient. Perform job with professionalism and good judgment. Work Environment and Physical Demands With multiple locations and settings, PCHS' work environment and physical demands vary greatly depending on the employee's role. Essential job functions include the following: The employee is required to talk, hear, see, smell, sit and stand.
