Job DescriptionFlexStaff is seeking a Nurse Case Manager/Staff Nurse to join our client's team, a leading organization that provides housing, services and programs for individuals with intellectual and developmental disabilities. This is a full-time (Mon-Fri), direct hire position, there are various group home locations in BROOKLYN. *Hours can be flexible 8-4pm, 9-5pm or 7am-3pm. Responsibilities:
- Coordinate the delivery of health care services for adults with disabilities residing in residential programs.
- Be responsible for performing nursing assessments and diagnoses
- Work in conjunction with other members of the interdisciplinary team to achieve positive health outcomes.
- Provides guidance and training to direct care staff to ensure proper delivery of health care services.
Requirements include:
- NYS RN License
- Prior nursing experience and/or experience working with persons with developmental disabilities preferred.
- Excellent oral and written communication skills.
- Ability to travel to and access multiple locations throughout the metropolitan area.
- Able to work flexible hours and/or days.
