FlexStaff is seeking a Nurse Case Manager/Staff Nurse to join our client's team, a leading organization that provides housing, services and programs for individuals with intellectual and developmental disabilities.
This is a full-time (Mon-Fri), direct hire position, there are various group home locations in BROOKLYN. *Hours can be flexible 8-4pm, 9-5pm or 7am-3pm.
Responsibilities:
* Coordinate the delivery of health care services for adults with disabilities residing in residential programs.
* Be responsible for performing nursing assessments and diagnoses
* Work in conjunction with other members of the interdisciplinary team to achieve positive health outcomes.
* Provides guidance and training to direct care staff to ensure proper delivery of health care services.
Requirements include:
* NYS RN License
* Prior nursing experience and/or experience working with persons with developmental disabilities preferred.
* Excellent oral and written communication skills.
* Ability to travel to and access multiple locations throughout the metropolitan area.
* Able to work flexible hours and/or days.
