Job Summary:

Serves identified members of select/partnered congregations as a nurse consultant in making health related choices and decisions, facilitating the most appropriate care. Escorts identified members to health appointments, attends appointments and assists in decision making. This may include driving the member to an appointment in a congregation owned vehicle.

Job Responsibilities and Requirements:

PRIMARY RESPONSIBILITIES

  • Coordinates quality care to acute, sub-acute, and chronically ill residents/tenants, according to standards of care, and maintains competency in all required nursing skills.
  • Accompanies members of Congregation of Sisters of Agnes to health appointments, facilitating appropriate care.
  • Operates vehicle in a safe manner when transporting residents/tenants. Informs supervisor of any unusual situations or accidents immediately. Assists riders entering and exiting the vehicle. Complies with all traffic safety rules and regulations.
  • Uses nursing process for: assessment, nursing diagnosis, planning, intervention, and evaluation. Report significant changes to Provider as indicated.
  • Gives guidance and direction to auxiliary staff. Participates in report meetings at the beginning and end of each shift. Communicates with residents/tenants, guardians, and family members on the current condition and needs of residents/tenants in accordance with regulations and standards of practice.
  • Acts as an advocate to resident/tenants by providing education to resident/tenants and answering questions in relation to their medical condition and ensures their understanding to each resident/tenant's ability.
  • Coordinates equipment needs in relation to appointments.
  • Coordinates/schedules appointments with Providers, including communicating laboratory findings as appropriate between Providers when needed.
  • Participates in Interdisciplinary meetings to review and identify any concerns and addresses any concerns for each resident/tenant. Participates in payer source meetings to coordinate the care of each resident/tenant with payer source.
  • Works in a constant state of alertness and safe manner.
  • Performs other duties as assigned.

EDUCATION

  • Graduate of accredited school of nursing or education equivalency for licensing

EXPERIENCE

  • Three years' experience

REQUIRED PROFESSIONAL LICENSE AND/OR CERTIFICATIONS State of Work Location: Illinois
