Endeavor Health Services is seeking a compassionate and dedicated Nurse Health Educator to join our dynamic team at the EAST Haven Crisis Stabilization Center. The Nurse Health Educator functions as a member of the EAST Haven interdisciplinary team by providing direct medical and educational services. The Nurse Health Educator will perform group-based health education, health screenings for people experiencing crisis, and health assessments for people with limited access to health care. The Nurse Health Educator will work within the team to connect people served to identified services such as primary and specialty care services.

Responsibilities include the following:

  • Provides preventative health and health literacy education in individual, family, and group modalities.
  • Collects vitals, including height, weight, blood pressure, temperature, and pulse oximetry.
  • Conducts the COWS, Point of Care BAC Testing, AUDIT-C, CIWA-AR, Fagerstrom Test for Nicotine Dependence, and other screenings as indicated and assigned.
  • Administers injections, collects urine samples, and draws labs, when needed.
  • Works in consultation with the EAST Haven counseling staff to triage a higher level of care for people experiencing a crisis presentation, if needed.
  • Consults with staff psychiatrist responsible for psychiatric and medication evaluation when needed.
  • Documents their encounters with people served, including a narrative about their presenting concern, any pertinent medical history, and will provide recommendations for treatment and linkage.
  • Concurrent documentation is required for all performed assessments.
  • Completes all required trainings in a timely manner, as assigned.
  • Obtains familiarity with current services, programs, locations, and specialty areas across Endeavor and external community providers.
  • Performs off-site service delivery, as assigned to meet the needs of the people served.
  • Maintains knowledge of community resources and assists with linkage and advocacy.
  • Maintains cultural humility and competence with respect to people served and their families, as well as other employees and suppliers.
  • Alerts Director of Crisis Stabilization of medical and laboratory supplies inventory to maintain supply.

Education and/or Experience

  • A bachelor's degree in nursing, currently licensed as a Registered Nurse by the New York State Department of Education.
  • Additional experience working and/or training in behavioral health is preferred but not required.

Salary starting at $38.00 - 41.00/hour. Flexible scheduling. 30 hours per week.
