Salary: Depends on QualificationsLocation : Weldon, NCJob Type: Full-TimeJob Number: NHVNFPDivision: Academic and Student AffairsDepartment: School of Allied Health and Career and Techical EducationOpening Date: 10/02/2024Description Nurse-Family Partnership (NFP) is an evidence-based community health nurse program that results in significant improvements in the health and lives of first-time families impacted by economic inequality. . The Nurse-Family Partnership National Service Office, located in Denver, Colorado is a nonprofit organization that provides service to communities in implementing and sustaining this program. Halifax Community College provides guidance and oversight for the Northeastern North Carolina Nurse-Family Partnership Program. Examples of Duties
  • Enroll, coordinate services, and provide comprehensive nursing care via in-person and/or virtual visits to a caseload of 25-30 NFP families. Visits may include nursing assessments, education, guidance, and support to families in their homes, locations of their choice, or via telehealth.
  • NFP nurses utilize self-reflective practices individually, during case conferences, and weekly self-reflection meetings with NFP supervisors or peers. Therapeutic relationships with NFP families are established utilizing the concepts of reflection and motivational interviewing.
  • NFP nurses partner with NFP families and other programs and community agencies to improve pregnancy outcomes, improve child health and development, and improve economic self-sufficiency.
  • NFP nurses attend and complete any in-person or virtually required NFP and/or HCC-related education, training, and outreach events.
  • The additional time 5%, will be duties and follow-up as assigned.
OTHER POSITION CHARACTERISTICSAccuracy Required in Work:
  • NFP nurses actively participate in continuous quality improvement activities.
  • Review and analyze data reports to determine outcomes and improvement achievements.
  • Data forms are completed accurately and recorded in the Clinical Information System (CIS).
The Consequence of Error:
  • Employees' work and decision-making could have a substantial impact on NFP families served, especially since this nurse may be the only health professional to encounter clients in the field when providing advanced nursing services.
Instructions Provided to Employee:
  • Daily and weekly work is self-planned.
  • NFP Nurses at HCC work from various locations to meet the needs of NFP families
  • Modality and location of NFP visits are completed based on collaboration between the nurse and the NFP family.

  • Nursing standards and policies as outlined by professional practice agencies.
  • HCC policies and procedures
  • NFP policies, procedures, guidelines
  • HIPAA privacy practices and confidentiality.
Supervision Received by Employee:
  • Work is reviewed by a nurse supervisor.
  • Onboarding process that includes NFP-required education and mentorship.
  • Joint NFP visits with the nurse supervisor are completed at least every four months.
Variety and Purpose of Personal Contacts:
  • Other disciplines that employees will have contact with include, but are not limited to, physicians, social service providers, community members, and stakeholders.

  • NFP at HCC nurses represent NFP and HCC's vision, mission, and model in actions and verbally with indirect and direct relationships.
  • Assists in creating a positive work environment that promotes understanding, productivity, mentoring, teamwork, and cooperation and recognizes accomplishments.
  • NFP nurses honestly communicate effectively, openly, and consistently with their families, colleagues, stakeholders, supervisors, and coworkers.
  • Creates, maintains, and supports a safe environment for discussions with NFP families and coworkers.
KNOWLEDGE, SKILLS, & ABILITIES
  • Considerable knowledge, skill, or experience in the following nursing fields: maternal child nursing, pediatric nursing, newborn care, or community health nursing.
  • Ability to record accurately services rendered and to interpret and explain records, reports, and medical instructions.
  • Ability to plan, coordinate, and utilize the nursing process along with Nurse-Family Partnership Guidelines.
  • Ability to deal tactfully with others and to exercise good judgment in appraising situations and making decisions.
  • Ability to secure and maintain effective working relationships.
  • Ability to record accurately services rendered and to interpret and explain records, reports, and medical instructions.
  • NFP nurses fundamentally enjoy practicing strength-based whole-person nursing while providing care to first-time families and their children up to 2 years old.
Minimum/Required Qualifications Required: Associate's Degree in Nursing and currently enrolled in RN to BSN program. Valid Driver's license. Current, active, non-restricted nursing license in North Carolina or Compact StatePreferred: Bachelor's Degree in Nursing is preferred.
