RN Nursing Supervisor Job Summary: Responsible for day-to-day supervision of direct client care, management of 3rd party payer authorizations, managing the weekly schedules of clinicians, taking the referral intakes, conducting the monthly clinical record reviews and participating in the monthly QA activities. Organizational Relationship: Reports directly to the Nursing Director Risk Of Occupational Exposure To Blood Borne Pathogens: B: limited exposure Qualifications: Must be a registered nurse, licensed in the state of MO in good standing. Must have at least two (2) years nursing experience and a background in public health, home health, or hospital work, including 1 year supervisory experience. Must have criminal background check. Must have current CPR certification. Multi-State License Preferred Spanish Speaking Preferred Responsibilities: • Takes referrals and assigns clients to field staff. • Follows up on referrals by contacting clients, physicians, family members, etc. • Handles client care supervision (day-to-day client care management). • Documents telephone and in-office conferences with physicians, clients /family, nursing staff, and significant others. • Manages the case conference process • Coordinates all services provided to clients. • Assists with interviews and hires select home care personnel. • Orients professional staff to clinical policies and procedures of our Agency • Participates in the Agency QA program. • Consults with physician when necessary to ensure and maintain safe, efficient, quality care. • Supervises all professional and non-professional staff to ensure quality performance standards. • Establishes mutual goal setting and achievement standards. • Projects a concerned, professional appearance toward departmental personnel. • Maintains high visibility and availability via telephone while in the office. • Works to develop a positive rapport/relationship with all staff members. • Carries beeper when away from office and responds within the hour. • Shares on-call responsibility on an equitable basis with staff members delegated by Nursing Supervisor. • Actively develops and pursues referral sources, as well as facilitates on-going relationships with various medical and health communities. • Works to develop and open positive rapport and relationships with community resources affiliated with home care services Functional Abilities: Must be able to hear and effectively communicate in English. Must be able to read 12 point or larger type. Must be able to travel to prospective clients' place of residence. Must be able to stoop and bend; must be able to lift and transfer clients. Licensed Practical Nurse (LPN) Job Summary: A Licensed Practical Nurse (LPN) provides practical nursing skills within the scope of nursing practice standards in collaboration with RN case managers/supervisors, teaches and educates clients and their families according to the established professional plan of care. Organizational Relationship: Reports directly to the assigned registered nurse Risk Of Occupational Exposure To Blood Borne Pathogens: A: high exposure Qualifications: Graduate of an approved LPN nursing school Current MO state license “in good standing” A minimum of one (1) year of experience as a licensed practical nurse, preferably in a home care setting. Must have a criminal background check Must have current CPR certification Multi-State License Preferred Spanish Speaking Preferred Responsibilities: • Maintains the nursing frequency established by the supervising RN. • Prepares equipment & materials for treatments observing aseptic techniques as required. • Follows the plan of care established by the supervising RN. • Performs home safety check and environmental check of the client's home environment on all visits. Reports any unsafe or potentially unsafe condition to the supervising RN. • Performs nursing visit and documents accordingly Uses care to always sign credentials anywhere a nursing signature is needed. • Reviews and follows the supervising RN's plan of care, communicating frequently with the supervising RN on any suggested changes necessary. • Reviews the client's ADL and iADL abilities and communicates any suggested changes to the supervising RN. • Effectively communicates with other disciplines in the case (actively participates in case conferencing) to effectively and appropriately problem solve as situations arise. • Communicates effectively with the supervising RN, scheduled visits planned and changes to the schedule on a weekly basis. Caseload is self-scheduled but communication of the clinician's schedule to the nursing supervisor is essential. • Communicates in case conferencing sessions. • Submits accurate, complete paperwork at the end of every week so that all records are intact and up to date. Uses the drop box if the office is already closed for the weekend. • Participates in the QA Committee. • Participates in the staff meetings as part of the requirements of the position. Adjusts visits on the staff meeting day to accommodate this important function. • Maintains LPN licensure and actively seeks out educational experiences to enhance the practice of home care nursing for her/him and the benefit of the clients serviced. • Notifies the Agency of emergencies, sickness, and other imminent occurrences that may affect the client caseload as quickly as possible relative to the event's occurrence. • Submits written time requests 2 weeks or more in advance of planned time off. Functional Abilities: Is able to lift, turn and transfer clients weighting up to 150+ pounds. Is able to carry bundles upstairs weighing up to 10 pounds. Must be able to stoop and bend. Must be able to travel to prospective client's place of residence. Must be able to hear and communicate effectively in English.
