POSITION SUMMARY/JOB PURPOSE:

The Nurse assists in the delivery of medication services to patients in Detoxification and Intensive Residential Treatment services and coordinates medical and non-medical aspects of a patient's treatment. This is a 24/7 facility with day, evening and night shifts available.

The Nurse rotates on-call as needed.

This position works independently, under limited supervision, reporting major activities through periodic meetings.

EDUCATION, EXPERIENCE, OTHER QUALIFICATIONS:

Education Required: High School Diploma or GED. Licensed Vocational Nurse (LVN) or Registered Nurse (RN) in the State of Texas.

Experience Required: At least 1 year of nursing experience.

Registration, Certification, Licensure or other Qualifications Required:

  • Must maintain a valid Texas Driver's license, auto liability insurance and a driving record acceptable to PermiaCare's insurance requirements.
  • Required to pass criminal history and background checks as well as pre-employment drug screen.
  • Current and valid Texas LVN or RN license.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Maintain standards of Nurse Practice Act where applicable.
  • Assess, monitor, and document health and medical conditions.
  • Keep supervisor, physician and/or others as specified informed of changes orally and/or in writing.
  • Document changes and treatments promptly.
  • Provide and document administration and monitoring of medications and treatments as ordered.
  • Obtain, follow and document doctor's orders.
  • Plan and secure health care services for individuals, as needed.
  • Educate, instruct, and train clients, family, and staff in medication, adverse reactions and side effects.
  • Maintain medication record book, medication cabinet and daily inventory of client medications.
  • Maintain client recordkeeping and documentation.
  • Monitor special medical devices, documents and coordinates repairs or replacements.
  • Transport clients to physical exams, ER, Planned Parenthood and all other medical needs.
  • Consult with the physician, ER doctors, and other health care professionals as needed.
  • Ensure that clients receive contractually required HIV/STD/TB/HCV, Hepatitis B and C, nicotine education, and Overdose Education.
  • Complete prescription medication record per Physicians Orders.
  • Rotate on-call as needed for admissions.
  • Meet unit performance measures or targets.
  • Enter accurate and appropriate documentation of services within timeframe required.
  • Maintain confidentiality of sensitive records and treatment information, client files and protected health information in compliance with HIPAA, laws, rules and regulations, and established procedures.
  • Maintain regular and reliable physical on-site attendance. Regular attendance, dependability, and promptness are required for the scheduled work day 100% of the time, to ensure consistency and completeness of program's processes.
  • Comply with the Abuse, Neglect, and Exploitation policy and reporting requirements.
  • Adhere to the Code of Conduct and Standards of Behavior policy requirements.
  • Establish and maintain effective work relationships with individuals served and their families, supervisors, co-workers and visitors by demonstrating cooperative, courteous and respectful behavior at all times.
  • Communicate regularly with supervisor.
  • Open and process mail/email in a timely manner.
  • Answer phone, collect phone messages and respond to requests timely and accurately.
  • Maintain safe and clean working environment by complying with procedures, rules and regulations.
  • Perform all work functions and interactions using a trauma informed approach.
  • Display professionalism when representing PermiaCare and the program in the community.
  • Maintain compliance with legal requirements and company policies and procedures.
  • Maintain valid and current driver's license, auto insurance, acceptable driving record and reliable transportation at all times. Driving may be required for this position.
  • Complete all training as assigned prior to due date.
  • Other duties as assigned.

  • Fill in for other nurses or Turning Point staff as needed.
  • Assemble equipment and/or transport equipment to provide for individual's needs.

  • Knowledge of Nurse Practice Act standards.
