Summary: The Licensed Practical Nurse will interact with the clients on a need-to-need basis. The LPN will work with the medical team, case managers, social workers and other health professionals to provide and ensure the needs of the clients' is met. The LPN is responsible to maintain the medication storage units. The work of the LPN will vary depending on the need of the program. Responsibilities:The Licensed Practical Nurse Responsibilities are but not limited to:
- Patient assessment while conversing with the clients
- Building open communication with clients to encourage trust, while listening to and interpreting their needs and concerns
- Ensuring the correct administration of medication by asking for ID cards
- Responding to distressed patients in a non-threatening manner and attempting to understand the source of distress
- Applying 'de-escalation' techniques to help people manage their emotions and behavior
- Preparing and maintaining patient records
- Develop and maintain effective working relations with colleagues
- Demonstrate respect for all team members, maximizing potential through the diverse roles within the team
- Attend team meetings as required and outside meetings on occasion
- Work closely with other team members to ensure that care is delivered to the highest standards
- Regular Consultation with psychiatrist, case managers, social services, and/or administrators
General Requirements:
- Communicate clearly and concisely in both written and verbal form, taking into account the ethical, professional and legal aspects of documentation.
- Document all aspects of care, including care provision and communication with relevant others.
- Manage time and resources effectively.
- Maintain and develop expertise in the acute care of people with a serious mental health problem, including the management of the people at risk of suicide.
- Develop and document Care Plans for individual clients, including discharge planning.
- Actively seek to improve practice by seeking new and innovative ideas for the delivery of care.
- Coordinating the care of the clients when leaving the shelter
- After care follow up with case manager
- Assessing patients' behavior and psychological needs
- Identifying whether and when patients are at risk of harming themselves or others
Professional & Personal Qualifications:
- Licensed Practical Nurse with minimum of five years in the mental health field
- Mental Health Certificate a plus
- Bilingual: Spanish or French
