PROGRAM BACKGROUND:To meet the needs of New York City's ever-changing foster care population. Our children have often experienced significant levels of trauma, poverty and violence. As they grow and mature, their individual needs change. Our assessments, health counseling, and selected treatments change to meet their new needs. Our children's physical and mental health as well as their strengths must be supported so they can reach their full potential.
POSITION SUMMARY:Under the supervision of the Nursing Supervisor, the LPN will be responsible for providing best practice nursing care to our clients. Our clients are a mix of infants, toddlers, school age children and teens and are male and female. LPNs will work with birth families, foster parents, and other members of each child's treatment team.
REQUIRED QUALIFICATIONS: Current Licensed Practical Nurse in New York State, current CPR certification and completion of required courses in Child Abuse and Infection Control. Must have and maintain a valid Driver's License with a satisfactory driving record.
RESPONSIBILITIES: - Conduct physical health assessments for infants, children, and adolescents including HIV Risk Assessments, General First Aid, Triage Activities for sick calls.
- Routine Screening for Substance Abuse, Developmental Health, Mental health, Physical Body Checks for Signs of Abuse
- Routine management and training regarding chronic conditions (Asthma, Diabetes)
- Take vital signs, heights, and weights; document observations and activities
- Provide immunizations and document same
- Provide first aid and emergency care as needed
- Execute medical orders (tests, tasks, hospitalizations, etc.) as ordered by physician/nurse practitioner/psychiatrist
- Obtain certain lab specimens
- Administer hearing and vision tests
- Training and Health Education related to Sexual Reproductive Health
- Medication Administration and Management Reconciliation
- Ensure required documentation for all case actions (medical notes) are completed within established timeframes.
- Maintain appropriate professional boundaries with clients and staff.
- Maintain a safe and secure medical environment for clients
- Accept medication deliveries, check for accuracy, store safely

SALARY RANGE: LPN: $30.00 - 32.00/hour. Full-time 40 hours/week.
