The Adult Day Health Care (ADHC) Program at client is a unique community-based health care program that provides comprehensive nursing, social work, nutrition, mental health and substance use services to individuals living with chronic illnesses including HIV, behavioral health conditions, diabetes, hypertension and others is seeking a Nurse Manager to serve as lead nurse, responsible for oversight of the nursing services at our locations, Manhattan and Downtown BrooklynType: Full-Time/ Direct-HireShifts: Mon-Fri 9a-5pLocation: New YorkSummary:This is a unique opportunity for community health focused Registered Nurse (RN) with clinical experience and management interest to work in a collaborative, outcome focused setting.The Nurse Manager is responsible for the leadership, direction, and oversight of nursing staff and patient care within a designated unit or department.This role ensures that nursing practices are following regulatory standards, patient safety protocols are followed, and that the delivery of care is effective, efficient, and compassionate.The Nurse Manager also plays a key role in budget management, staff development, performance improvement, and fostering a positive working environment.With the support of the Chief Medical Officer, Program Director and an interdisciplinary team of professionals, the Nurse Manager will help achieve improved health outcomes for our client population.The ideal candidate will be familiar with population health model and comfortable using data to guide practice.Responsibilities:Leadership & Staff Management:* Provide direct clinical and administrative oversight of program nurses.* Coordinate care between ADHC program and clients' primary care provider* Ensure adequate nursing coverage across program locations.* Ensure program nursing assessments are completed and that program nursing services are delivered in a manner compliant with NYS DOH nursing standards.* Provided direct nursing services (individual and group interventions)* Develop staffing schedules to ensure adequate coverage to manage workforce productivity* Assist with the development of effective interventions and treatment plans* Monitor program compliance and quality assurance* Participate in agency wide continuous quality improvement activities aimed at improving health outcomes for program participants* Lead the development and evaluation of individual and group level nursing interventions* Collaborate with interdepartmental population health interventions to improve the overall health of the client community* Provide mentorship, training, and professional development opportunities for nursing staff, with a focus on geriatric care principles, including managing chronic diseases, dementia care, and end-of-life issues.* Develop strategies to manage chronic conditions, improve functional status, and enhance quality of life for elderly patients.* Participate in external workgroups to strengthen the ADHC model.* Lead, manage, and supervise nursing staff, ensuring high levels of performance, teamwork, and patient-centered care.* Lead quality improvement interventions and reporting findings to the team* Familiarity with the use of community health education to promote health outcomes.* Develop staffing schedules to ensure adequate coverage and manage workforce productivity.* Conduct regular performance evaluations and provide constructive feedback to staff.* Foster a supportive, inclusive, and collaborative environment within the nursing team.Operational Management:* Manage the daily operations of the nursing unit, including patient flow, resource allocation, and overall efficiency of the nursing staff* Monitor the needs of the team, including supplies, equipment, and staffing needs, to maintain financial sustainability.* Ensure compliance with regulatory bodies, accrediting agencies, and agency policies and procedures.* Develop, implement, and monitor performance improvement initiatives to enhance operational efficiency.Collaboration & Communication:* Communicate effectively with the leadership team, other departments, and external stakeholders.* Participate in agency committees, quality improvement meetings, and other leadership functions.* Serve as a liaison between nursing staff and program administration to relay information and advocate for needed resources.Professional Development & Education* Stay current with nursing trends, regulations, and healthcare innovations to ensure high-quality care.* Encourage and support continuing education and certification opportunities for nursing staff.* Provide clinical guidance and training to nursing staff as necessary, including on new technologies, treatments, or procedures.Minimum Requirements* Bachelor's degree in nursing (BSN) required; master's degree in nursing or healthcare administration preferred.* Current, active Registered Nurse (RN) license in the state of practice.* Minimum 5 years of clinical nursing experience, with at least 2 years in a supervisory or leadership role.* Experience or sensitivity to working individuals or families who have issues of mental illness, substance use, homelessness or HIV/AIDS* Experience working with patients in the geriatric modality* Strong leadership ability* Ability to use data to inform clinical and administrative decision making* Ability to apply population health and Continuous Quality Improvement principles to work* Bilingual candidates encouraged to applyPlease email: ...@hcmmi.comSalary: $95,000-$105,000#LI-PA1
