Job Description:

Responsible for serving as the first reliable point of contact for Patients, Families and Providers.Principal Accountabilities:Serves as patient and family advocate.Coordinates the scheduling of appointments to expedite the plan of care and to promote continuity of care.Schedules, re-schedules, and cancels appointments for the appropriate outpatient service.Assists in education of the patient/family to understand disease process and care map.Assists in answering patient calls, initiate call backs and follow-up to meet the needs of the patient.Assists with communication to internal departments and external contacts for insurance verification and authorization process.Supports a smooth transition of patients from outside referrals to internal referrals for appropriate treatment.Performs other related duties as assigned.Required:Bachelor's in Nursing or related field.Valid State or national Nursing License.2 years of experience in Acute Care or relevant area.Preferred:BSN preferred.Atlantic Medical Group is a physician-led and physician-governed organization that delivers the highest quality health care, at the right place, the right price, and the right time.
