Job Description:

The nurse navigator will coordinate the patient#s care to increase the efficiency and quality of care that is delivered. This role is initiated at time of diagnosis and will continue through the patient#s journey,#being a consistent contact at each critical touch point including new treatments, surgery, radiation, clinical trials, hospitalization, survivorship, etc. The nurse navigator is a patient advocate who serves in a highly supportive and personal manner and also ensures that patients and families receive resources and services as needed. Effective communication, organization, compassion, and providing individual attention to patients are all critical to the success of this position. PRIMARY DUTIES: Contacts every patient at the time of diagnosis and provides an introduction for staff and providers, sets expectations for initial consultations and treatments, and outlines the services provided within the cancer center and GVMH related to their diagnosis. Provides diagnosis specific education, identifies and coordinates resources, and reviews treatment options following the initial consultation. Collaborates with patient and providers to develop individualized short-term and long-term goals. Provides ongoing assessment of patient care management needs through frequent communication with providers and keeps others on the care team updated on patient status, treatment plan, and patient goals. Routinely collaborates with outside facilities and various medical professionals to coordinate all aspects of patient care. Routinely assesses patients for support needs including: financial burden/health insurance, transportation, therapy, occupation gaps,#survivorship, and family/caregiver support, etc., and coordinates with the appropriate resources to eliminate barriers where possible. Obtain appropriate records and test results for proper patient evaluation, reports significant patient conditions to patient#s physician promptly, and properly documents each interaction with the patient to provide appropriate continuity of care. ##################################################### ################################### Lends support and advocacy during consultation with a surgical oncologist, medical oncologist, and/or a radiation oncologist. Provides compassion and support during unexpected hospitalization, complications, or setbacks. Will also be trained in clinic work flows for all cancer center providers in order to further support clinic services at any of our locations as needed. Required: Current Missouri RN license in good standing BLS (or ability to obtain within 30 days of hire) Willing to obtain chemo/biotherapy administration certification within six months of hire One to three years# nursing experience # #*Starting Range: $31.28-$39.10 Our competitive salary package is adjusted to align with the candidate#s relevant experience and skills.
